My loved one was admitted on a 1013 (involuntary evaluation) for severe alcohol abuse. He was kept in the hospital E.R. for approx. 2-3 hours and then discharged and sent home. He was found deceased in his home exactly one week later. Neither the attending physician in the E.R. nor the counselor from Laurelwood ever saw our family's written Affidavits, nor did they request them from the Probate Court! DO NOT TRUST THEM TO PROVIDE THE NECESSARY HELP FOR YOUR LOVED ONE!!!
An Authorized Person is someone who is formally and properly empowered to act on behalf of Laurelwood - Northeast Georgia Health System
