I am writing to express my deep concern regarding the conditions and management at your facility. After a recent visit and interaction with staff, including the administrator, Nancy, I was left genuinely troubled by the level of care and professionalism—or lack thereof—being provided. As someone who has worked in healthcare and visited numerous facilities over the years, I found the environment at your facility to be unacceptable by any reasonable standard. Frankly, I’m not sure how this place remains operational. The way individuals are treated is not only disappointing but borders on shameful. When I voiced my concerns to Nancy, she appeared surprised by my statements, which only further underscores the disconnect between leadership and the reality on the ground. I am seriously questioning what kind of oversight or inspection process is in place here, because from what I witnessed, this facility falls far below industry standards—and in my opinion, should not be in operation in its current state. I hope this message is taken seriously, as it comes from a place of genuine concern for the well-being of those in your care. I would appreciate a response outlining any steps that are being taken to address these issues, or at minimum, acknowledgment that this message has been received.

The Guardian Center was a great help in my father's recovery post stroke. Nancy Zraizza and the staff were amazing assets and cared for him like he was family. Lynda McIntosh and Lucy Brandao went above and beyond.

I cannot make enough positive comments regarding the care my husband has received while rehabbing at the Guardian Center. Under the guidance of their leader, Nancy Zraizza, the staff has made every effort to develop the sense that our loved one is part of their family. He has been treated with dignity and respect. Special shout out to aides Lynda McIntosh and Lucy Brandao for their gentle care and cheerful vibe.

Levels of Care

Residential treatment programs are those that offer housing and meals in addition to substance abuse treatment. Rehab facilities that offer residential treatment allow patients to focus solely on recovery, in an environment totally separate from their lives. This place almost killed my mother stole from her and then denied she was ever there when others asked about the situation this place needs to be shut down for how they leave patients sitting for over an hour before getting life saving help to them shame on this horrid place I will be contacting the necessary governmental agency to have it looked into asap!
