The charge nurse Jana and the other blonde nurse in the morning on 1 north have to be the pair of most unprofessional nurses I have ever come across. Both are extremely rude and dismissive towards everyone. Many patients and families have said the same. I wonder how Jana got her license, must've been down in florida. She comes into work already upset and not wanting to do her job.

I was here for about a month getting IV treatment for sepsis. All of the nurses on the second floor, Virginia’s crew, were very attentive to my needs. Being in a hospital bed for quite a while and had serious back pain. Virginia is in charge and she is right on top of everything! Pete, the floor doctor/nurse was fantastic also. Any questions I had medication wise, he was quick to answer them. The ladies in PT came up to my room every day, even though it was very difficult for me to get out of bed and come down to the gym and push myself. There was some days I could not get out of bed, but they were persistent and fantastic also. Shantel The director of social services was very friendly and accommodating also. I don’t care who you are. It’s not a great experience being In rehabilitation for that long. But the entire staff are true professionals, and I respect every one of you. And everybody who works here tries to make your life a little easier. I would’ve gave five stars except for the food! LOL.

Thank you, Sean, for sharing such a detailed and heartfelt review of your experience at Affinity Skilled Living and Rehabilitation! ⭐⭐⭐⭐⭐ We're so glad to hear that Virginia and her second-floor nursing team provided attentive care throughout your IV treatment and recovery. Pete’s responsiveness and expertise clearly made a strong impression, and it’s great to know the physical therapy team consistently encouraged your progress, even on the hard days.

Shantel’s support from the social services side rounds out what sounds like an incredibly dedicated and compassionate staff. Your honesty about the food gave us a smile. Thanks for keeping it real! Wishing you continued health and strength, and thank you again for recognizing the effort and professionalism of the Affinity team. 💙

A letter from the Piffard family to Affinity: To Whom it May Concern, In the final chapter of my father, Charles Piffard’s life, we were blessed with the care and compassion of an extraordinary team at Affinity. There were a few members of the team that really made a difference. RN’s Jenn Dowling and Pedre Pierre, alone with CNA’s Maleita Osbourne and Elsie Lindor, cared for my father not just with medical skill, but with humanity, patience, and deep respect. Jen, Pedro, Maleita and Elsie each played a vital role in helping my father maintain his dignity as his strength faded. Their attentiveness, kindness and quiet strength brought comfort not only to him, but to our entire family. They treated him as a person, not just a patient- with kindness and compassion Each of them has a heart of gold. We are forever grateful for their care, their grace, and the way they honored my father’s life through the tenderness of his care. He told me before he passed that it was important for him to have been at Affinity because it was there that he learned the love of strangers. The team’s dedication went beyond duty, in time of grief, their presence was a blessing; the kind that leaves a permanent mark. We will be forever grateful. With Warm Regards, The Piffard Family

Accepted Insurance

Affinity Skilled Living and Rehabilitation works with several private insurance providers and also accepts private payments when possible, please contact to verify your specific insurance provider.

Other Forms of Payment

Private insurance refers to any kind of healthcare coverage that isn't from the state or federal government. This includes individual and family plans offered by an employer or purchased from the Insurance Marketplace. Every plan will have different requirements and out of pocket costs so be sure to get the full details before you start treatment.

Self-pay involves paying for treatment out of your own pocket. You can use savings or credit, get a personal loan, or receive help from family and friends to fund your treatment. If you don't have insurance or your insurance plan doesn't cover a specific program, self-pay can help ensure you still get the care you need.

Financial aid can take many forms. Centers may have grants or scholarships available to clients who meet eligibility requirements. Programs that receive SAMHSA grants may have financial aid available for those who need treatment as well. Grants and scholarships can help you pai for treatment without having to repay.

Sliding scale payments are based on a client's income and family size. The goal is to make treatment affordable to everyone. By taking these factors into account, addiction recovery care providers help ensure that your treatment does not become a financial burden to you or your family, eliminating one barrier to care.

Medicare is a federal program that provides health insurance for those 65 and older. It also serves people under 65 with chronic and disabling health challenges. To use Medicare for addiction treatment you need to find a program that accepts Medicare and is in network with your plan. Out of pocket costs and preauthorization requirements vary, so always check with your provider.

Military members, veterans, and eligible dependents have access to specific insurance programs that help them get the care they need. TRICARE and VA insurance can help you access low cost or no cost addiction and mental health treatment. Programs that accept military insurance often have targeted treatment focused on the unique challenges military members, veterans, and their families face.

Medicaid is a state based program that helps lower-income individuals and families pay for healthcare. Medicaid covers addiction treatment so those enrolled can use their coverage to pay for rehab. When a program accepts Medicaid the client often pays very little or nothing out of their own pocket.

Addiction Treatments

Levels of Care

outpatient iconOutpatient

Outpatient Programs (OP) are for those seeking mental rehab or drug rehab, but who also stay at home every night. The main difference between outpatient treatment (OP) and intensive outpatient treatment (IOP) lies in the amount of hours the patient spends at the facility. Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

heart-hands iconIntervention Services

A drug intervention in New York occurs when family members and friends lovingly confront someone with a substance use disorder. The goal of this confrontation is to motivate the person to seek treatment. Loved ones point out how the person's destructive behavior is affecting them in hopes that they will agree to get help. Most inpatient rehab facilities offer intervention services that can help families walk through this process step by step.

24-hour icon24-Hour Clinical Care

For many different stages and therapies offered during addiction treatment, it's essential to offer 24-hour clinical care in New York. often involves medication treatment for drug or alcohol detox. The withdrawal symptoms experienced during detox can be dangerous, but having medical experts on-site around the clock means someone is always there to step in and provide emergency medical care when and if it is ever needed.

Treatments

Many of those suffering from addiction also suffer from mental or emotional illnesses like schizophrenia, bipolar disorder, depression, or anxiety disorders. Rehab and other substance abuse facilities treating those with a dual diagnosis or co-occurring disorder administer psychiatric treatment to address the person's mental health issue in addition to drug and alcohol rehabilitation.

Mental health rehabs focus on helping individuals recover from mental illnesses like bipolar disorder, clinical depression, anxiety disorders, schizophrenia, and more. Mental health professionals at these facilities are trained to understand and treat mental health issues, both in individual and group settings.

Programs

adult-program thumbnail image

Adult Program

Adult rehab programs include therapies tailored to each client's specific needs, goals, and recovery progress. They are tailored to the specific challenges adult clients may face, including family and work pressures and commitments. From inpatient and residential treatment to various levels of outpatient services, there are many options available. Some facilities also help adults work through co-occurring conditions, like anxiety, that can accompany addiction.
young-adult-program thumbnail image

Young Adult Program

Young adulthood can be an exciting, yet difficult, time of transition. Individuals in their late teens to mid-20s face unique stressors related to school, jobs, families, and social circles, which can lead to a rise in substance use. Rehab centers with dedicated young adult programs will include activities and amenities that cater to this age group, with an emphasis on specialized counseling, peer socialization, and ongoing aftercare.
military-program thumbnail image

Military Program

Serving in the military is both mentally and physically challenging, and can result in trauma that persists even after combat ends. Military programs are tailored to the specific and often complex needs of active duty personnel, veterans, and military families. Clients often access these programs through the U.S. Department of Veterans Affairs (VA).

Clinical Services

Cognitive Behavioral Therapy (CBT) is a therapy modality that focuses on the relationship between one's thoughts, feelings, and behaviors. It is used to establish and allow for healthy responses to thoughts and feelings (instead of unhealthy responses, like using drugs or alcohol). CBT has been proven effective for recovering addicts of all kinds, and is used to strengthen a patient's own self-awareness and ability to self-regulate. CBT allows individuals to monitor their own emotional state, become more adept at communicating with others, and manage stress without needing to engage in substance abuse.

Group therapy is any therapeutic work that happens in a group (not one-on-one). There are a number of different group therapy modalities, including support groups, experiential therapy, psycho-education, and more. Group therapy involves treatment as well as processing interaction between group members.

In individual therapy, a patient meets one-on-one with a trained psychologist or counselor. Therapy is a pivotal part of effective substance abuse treatment, as it often covers root causes of addiction, including challenges faced by the patient in their social, family, and work/school life.

Trauma therapy addresses traumatic incidents from a client's past that are likely affecting their present-day experience. Trauma is often one of the primary triggers and potential causes of addiction, and can stem from child sexual abuse, domestic violence, having a parent with a mental illness, losing one or both parents at a young age, teenage or adult sexual assault, or any number of other factors. The purpose of trauma therapy is to allow a patient to process trauma and move through and past it, with the help of trained and compassionate mental health professionals.

Research clearly demonstrates that recovery is far more successful and sustainable when loved ones like family members participate in rehab and substance abuse treatment. Genetic factors may be at play when it comes to drug and alcohol addiction, as well as mental health issues. Family dynamics often play a critical role in addiction triggers, and if properly educated, family members can be a strong source of support when it comes to rehabilitation.

As you recover from addiction, eating the right foods will help you heal and build your health for long term sobriety. Nutrition therapy teaches you about the nutrition your body needs and how to eat the right foods to support your recovery.

In addiction treatment, recreational therapy focuses on engaging you in healthy activities to help reduce your cravings and improve your mental well being. You might engage in activities like cooking classes, hiking, group sports, or swimming as a constructive outlet for stress and emotions that help your recovery.

When recovering from addiction, it can be difficult to find the words to express your emotions and thoughts. Creative arts therapy in New York gives you a way to express yourself beyond words. Options include sculpting, music, drawing, dance, and drama.

Experiential therapy is a form of therapy in which clients are encouraged to surface and work through subconscious issues by engaging in real-time experiences. Experiential therapy departs from traditional talk therapy by involving the body, and having clients engage in activities, movements, and physical and emotional expression. This can involve role-play or using props (which can include other people). Experiential therapy can help people process trauma, memories, and emotion quickly, deeply, and in a lasting fashion, leading to substantial and impactful healing.

Horrible place for your love ones, please consider before leaving them here! Food is disgusting , there no hygiene as well as how they treat the patient’s my relative lasted 1 week and came back with a huge bruise on his arm and complaint that they hit him inside, they were also giving him medication that that the dr had discontinued and antidepressants without patient or healthcare proxy’s consent! I have been here 3 weeks and the care is abysmal. The housekeeping is poor, the upkeep is non-existant with plaster patchwork, holes in the walls, the furniture is not clean. Do NOT expect the website photos to be an accurate depiction. I was also treated roughly by an aide. I'm only here for rehab which is good but it doesn't make up for the other shortcomings. I would nit recommend. My dissatisfaction and complaints have only generated worse care.

Response from the owner
Thank you for taking the time to share your thoughts, Mary, and we are sorry to hear that your experience has not been as expected. We care deeply about the well-being of all our residents and would appreciate the opportunity to speak with you personally. Please reach out to us so we can better assist you.

Terrible food no nutritional value, short handed at night. Took 1.4 hours to get my pain meds. Never get a straight answer or a yes (not exaggerating). Don’t come here doesn’t seem to have any type of training or process for nurses aides . I have not been allowed to shower for 4 days cause I missed my day, now it’s not a busy area but the nurse keeps saying they have to stick to the schedule but there’s no reason when it is empty 85% of the day. Just don’t want to do anything extra and never get an answer, just for profit.

Response from the owner
