There aren't enough ways to say thank you for all that they have done for my father and in turn my family. Ok, let's start with the fact they went above and beyond to care for him while he was there. Attentive, caring, and consistent throughout. Constant updates from them and they listened about concerns when we spoke. I'll finish with after he was discharged they still managed to care for him by helping us with his prescriptions when his primary wouldn't. Amazing, wonderful, and beyond helpful. ❤️ Huge shout out to Rita the Asst Director of Nursing, Dawn, Josephine and of course every other angel whose name I cannot remember right now, thank you for bringing my dad home to us.

Levels of Care

Inpatient

Residential treatment programs are those that offer housing and meals in addition to substance abuse treatment. I’m very grateful for this Rehabilitation center, And for all the help and support I have received during this difficult time, from the day I got here after having spinal surgery, I thank God for each and every person who showed me so much kindness , All the nurses are very kind and professional. I wanted to share my experience here at Pawtucket Falls Healthcare Center. I arrived on the evening of the 22nd after being discharged from Miriam. When entering my emotions got the best of me all I could do was cry because I never thought I would experience something like this. I came here cause I couldn’t walk, get out of bed, and was always falling. Once I was in my own room and settled down I was welcomed by Ana and Ella they were very nice. The next couple of days I was introduced by the physical therapist Michael. My goal was to work with physical therapy and get better so I could go home and be with my 5 kids. The first thing we did was the bicycle and that was a challenge for me. We continued with that and then we moved to the stairs. Now let me tell yall that was the hardest thing only because my legs were very weak and I didn’t think I could do it. Moving further I was introduced to the stairs and I was dreading it cause I knew it was going to be very hard. I tried to do the stairs every chance I could get along with the bicycle. I’ll say the stairs was my biggest challenge but I wasn’t going to let it get the best of me. I feel confident when being discharged I will still continue to do the things I did here. The staff here as well as the care was phenomenal nothing I ever experienced. I would definitely recommend this place for anyone and you won’t be disappointed. I wanna say thanks for all the support and love this place has given me because without them I’d probably would be home withering away in bed. Yall are the absolute best.

I was at the facility for short term and my overall experience was amazing. The staff was amazing. I specifically enjoyed the first shift staff, they met all my needs and helped assist me in my time of need. Overnight staffing was a bit short but it did not stop the nursing staff from meeting my needs. The therapy staff is AMAZING. 10/10 for meetjng all my therapy needs The food is alright. I would definitely return here and highly recommend coming here.

If I could give this facility negative stars, I absolutely would. My father almost died due to their lack of knowledge. I don’t know where the nursing staff received their education, but they definitely need more training. When an individual's hemoglobin is at 6.8, that is dangerously low. According to the staff at this facility, it isn’t considered dangerous until it drops to 6.5. On top of that, they were giving my father sodium tablets, which is not recommended for someone with congestive heart failure. I know this for a fact. Not only did they cause my father to experience fluid buildup, but he also required a blood transfusion. Additionally, there were multiple hematomas on his back from falls, yet when they called to inform me, they stated that there were no visible marks. They also didn’t properly weigh him, even though it was a standard order due to his congestive heart failure. This is just a brief overview of the medical problems he faced there. Let’s not forget the filthy conditions of the facility—there were fruit flies everywhere. I even left a creamer on the floor to see how long it would take for it to be picked up; after three days, I retrieved it myself. The food was inedible and disgusting; I can't fathom how anyone could find it acceptable. I could go on and on. I would not recommend this facility to anyone, not even an ant, let alone a human being.

This building has come a long way. The leadership team at the facility has taken it to the next level! Clinical capabilities have increased, including their brand new dementia unit. Very impressive.
