I would give them no stars. This is the worse place ever!!! They don’t answer the phone. Nurses are rude. They fail to take care of the residents. Don’t sent your family there! I called the state board several times

I am currently a patient at this place it’s 1am no one is seen or heard from for hours patients are screaming crying out for help but nothing no response we bed bound can’t move because of injuries we are at their control it’s a shame

I really despise this facility. My mom died here last month and the people here showed no compassion. My sister and I received phone calls in the morning stating that my mom was unresponsive. Two family members arrived shortly after seeking information to where my mom was and a nurse at the nurse’s station quickly responded I think that’s the lady that died. Total lack of compassion. The two family members proceeded to my mom’s room to uncover her lying on the bed lifeless and exposed indecency. They didn’t even have the sympathy to cover her up knowing family members will be coming soon. Weeks before my mom’s passing she was sent to the local hospital for two days. She told me that being at the hospital were the best two days she has had recently. The food was decent, the nurses were attentive and not gossiping amongst each other, and the atmosphere was overall amazing. However, arriving back here she stated that she automatically felt depressed as soon as she arrived to the so called rehab parking lot. I beg anyone who is thinking about sending their love ones here to second guess it!! They do not care. All they want is a pay check. Barely want that because half of the staff does not show up for work. I deeply suggest that this facility be looked into. Please!!!

Adding my experience with this facility. To be clear, my experience was when the name was "golden living center". I deliberately used lower case letters to type the name as the facility is not worthy of proper grammar. Both of my parents ended up at this facility for very different reasons, several years apart. Both of them were neglected. My father was an example of a patient they'd wheel to the area near the nurse's station for the day - nurses so lazy they'd wheel everyone to the station, strap them in their wheelchairs. Nurses didn't have to bother to stand up, let alone walk to patient rooms. My father died in that facility. I voiced concerns with the executive director at the time, was given lip service from him regarding promises to correct, which never materialized. My father was neglected to the point he suffered dehydration followed by respiratory distress. He died suffering. My mother died in this facility - poor quality care, similar circumstances. It was so unfortunate my parents both ended up there for reasons I had no control over in the moment of admission. They both died within EIGHT WEEKS of admission. There's a special place in hell for the people allowing these things to happen to the helpless. There ARE some who give a damn about patient care and wellness. But they are so few and far between at this facility, their efforts are futile. Note the facility has changed hands multiple times over the years - writing on the wall, folks. Don't let what happened to my precious parents happen to your loved ones. If they have to be admitted to this place, immediately start seeking an alternative. Only - and I mean only - allow a loved one in this place if there are no other options and only for as short of a period as possible.

Please do not send your Love one to this facility. I sent my mom here for a year and so and within that year, so she stopped walking and talking I used to come sometimes and her food would be sitting across the room and she was non-mobile her roommate was scared to tell me stuff she told me that the nurses were stealing my mom’s stuff and giving it to other patients. Every time I came my mom had a bruise on her head. I continuously asked them to help my mother eat. They told me I had to fill out some kind of paperwork. Somehow, my mom, being in a wheelchair, she kept falling. I have a lot of pictures of her Forehead she was rushed to the hospital more than five times. My mother’s hospice nurse suggested that I called the state. Also, they put my mother stuff in the back where the deceased patients are, and she wasn’t dead yet she was in the hospital. I asked them for her medical records last year, February 19 and it’s a year later no Medical records or anything. One day, I even was searching for my mom and she was sitting in Another patient’s room. Her roommate wanted to tell me stuff, but every time I came Standing there, making sure that she went and tell me anything. This lady Actually told me and my sister that she was afraid to tell us anything because they were gonna treat her differently. I am now in the works of starting an investigation towards this facility. my mother was severely neglected to the point where her hospice nurse wanted me to report their facility. I called three times and I couldn’t get no help. My mother died February 19 and I feel like it’s solely the nursing homes fault I wish I would’ve never put her in this facility because she would still be alive. I have photos and pictures if anyone wants me to email them to them please do not put your family member in here, especially if you aren’t checking on them every day.. I took these photos two months before I drop my mother off to these people, and these are the photos that I took when they called me and told me that she kept following which I don’t understand because she was in a wheelchair they were letting her fall asleep in the dining room

This place has helped me out so much with my recovery and My focus is so much better now and I wanna say Thank You for being so good to me!! Some things have changed now it's July 17th 2024 makes 7 weeks ago that they started to fix a leak in the shower on the 4th floor and this is what progress they have made on it so far. My question is, how does 70 plus people maintain hygiene?? On another note I wanna thank The Kitchen Staff for doing such a great job cooking because I appreciate it!! I also wanna thank The House Keepers for doing such a great job in keeping this place clean and Thank You for doing my Laundry because I really appreciate it!! I also wanna Thank The Nurses, CNA's and RCA's for helping me out and for doing such a great job 👍. The new showers on the 4th floor are very nice they did a great job on the remodel 💯💯!! Thank You

This is the worst place. It is run like a prison. Residents have no rights. Showers are constantly broken. If something in the room is broken, it will not be repaired. The meals are atrocious. The staff will fight with each other in the hall very loudly. Medication is not passed out on any schedule. The laundry has just lost people's clothes. Concerns fall on deaf ears. Their go to reason for just about everything is "because I said so". Don't come here.

If I could give this place -100 stars, I would. They don’t care and are extremely Incompetent! My sister had been here since May 2023. She is vegan/Vegetarian. How is it that they continue to bring her ham pork chops beef daily for her lunch and sometime don’t feed her to the point to where every day I have to pay to have food sent to her or drive over an hour to take her food and they’re paid over $2000 a month for her to reside there. Most recently I spoke to my sister, which is diabetic it was two in the afternoon. My sister had not eaten because for breakfast they brought her bacon again, but she does not eat eggs, but she does not eat. I spoke with the staff. They said they were on the way bringing the food at 2 PM at 2:22. They still have not made their way to her room which forced me to have to order food for her when they did bring her food they bring her green beans, peas, and mashed potatoes and a Styrofoam to go container no type of Preparation or care for the food. Her bed is burn broken, which should take them several months to fix. Also, most recently she ended up in the hospital from having a UTI. How are you inside of a facility with medical staff and getting a UTI that’s ridiculous. They didn’t even want her to call 911 to take her to the hospital because she continued to tell them she didn’t feel good and the staff became upset with her when she did call 911. Whomever is in charge of this place should really think about letting the entire staff go business staff included kitchen staff. There are a handful of nurses that are that are excellent staff there but everyone else they need to fire them and start over again all the way from the top damn. When you enter the facility, it smells of urine. My sister‘s roommate is absolutely intolerable. The room is dirty. The floor is dirty. It’s just absolutely filthy and Disgusting! I will be removing her from this facility very soon!

Do not let your people go here. They have let my mother personal belongings disappear. Clothes, laptop, wheelchair, and more. Your love one will develop bed sores and if you don’t have money or insurance they will let your love ones die. It old and needs renovation or something. I hate that my mom is here. But we’ve got a lawyer and we are planning to sue. Do not let your love ones get stuck here.

Our loved one was admitted to the facility about a year ago, and our experience has been nothing but positive all through. At the time of admission the hospital where our family-member was discharged from told us that he was not expected to live longer than a few weeks or three months; however; he has been at the Perimeter Rehabilitation Suites for 15 months now, and he continues to make progress. The lead nurse, the respiratory therapist, the speech therapist, the occupational therapist, and the nurses have been very caring and supportive professionally and humanely. Our loved one is lucky to have been placed with the Perimeter Rehab, and we are very appreciative of the care that he has been receiving from the staff of the facility. Kudos to the management of Perimeter Rehabilitation Suites by Harborview.

First impressions truly are lasting. Despite the negative reviews I read online, my initial visit to this facility was a pleasant surprise. From the moment I walked in, I was greeted warmly by the receptionist, which immediately set a positive tone. The facility was clean, and as I walked past at least seven rooms on the way to visit my family member, there were no unpleasant odors—a detail that did not go unnoticed. When I arrived at my cousin’s room, who is recovering from a stroke, despite his weakness, I was thrilled to find him clean, fresh, and truly shining like glass. The level of care was evident, and it brought me immense relief to see him so well-tended. On my way to the nurses’ station to meet his nurse, I passed a group of female patients chatting in the hallway. They were well-groomed and radiated warmth as they welcomed me into their conversation as if we were old friends. I absolutely love senior citizens, and their joy and camaraderie touched my heart deeply. When I spoke with the nurses at the station, they greeted me kindly and directed me to Nurse Tina, who was assigned to my cousin's care. Tina was busy at the computer, but she paused her work to address my questions with professionalism and warmth. Her dedication to both her tasks and the families she serves really stood out to me. Since that first visit, I have returned several times, and I’m happy to report that my experiences have been consistently positive. While I understand that everyone’s experience may differ, I felt it was important to share my perspective because it contrasts with some of the reviews I initially read. We often rush to report negative experiences but overlook the positive ones. The staff at this facility, especially Ms. Tina, Princess and Catherine deserve recognition for their hard work and compassionate care. Kudos to the entire 3rd floor team for being the true pillars of care and support. In a world that is troubled and divided there is profound need for unity and kindness. Keep up the fantastic work—you are making a difference!
