About Northeast Florida State Hospital

Northeast Florida State Hospital is a 613-bed state-owned inpatient psychiatric facility in Macclenny, Florida, 35 miles west of Jacksonville. Operated by the Florida Department of Children and Families and serving a 30-county region of northeast Florida, the hospital has provided continuous care to people with severe and persistent mental illness since 1959. Inpatient Psychiatric and Forensic Mental Health Care

Northeast Florida State Hospital accepts adults 18 and older who have been committed through civil or criminal justice proceedings. The Adult Forensic Mental Health (AFMH) program serves individuals involved in the criminal justice system who also carry a mental illness diagnosis, including those found not guilty by reason of insanity or deemed incompetent to proceed with criminal proceedings. The program provides comprehensive evaluation, assessment, and inpatient psychiatric treatment, as well as specialized modules covering anger management, competency restoration, and mental health awareness.

Many people admitted to the hospital live with co-occurring substance use disorder alongside a primary mental illness diagnosis. The hospital addresses both through integrated dual diagnosis treatment, with substance abuse awareness and prevention offered as a dedicated component of the forensic program. Clinical services include individual therapy, group therapy, Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), trauma therapy, and nutrition therapy.

Recovery-Centered Approach and Accreditation

Treatment follows a person-centered model. Upon admission, each individual receives a service plan built around comprehensive evaluations, clinical observations, and the person’s stated strengths, needs, and goals. Psychosocial rehabilitation is delivered through the Treatment Mall and Recovery Centers on campus, with a focus on life skills, social connection, and preparation for community reintegration. The hospital also provides comprehensive medical and dental services, occupational therapy, and nicotine replacement therapy.

Northeast Florida State Hospital holds continuous CARF accreditation since 2009 and has received the Governor’s Sterling Award for Organizational Performance Excellence. The facility accepts Medicaid, Medicare, private insurance, self-pay, and financial aid. Admissions are based on civil or forensic commitment and community referral priorities for the 30-county northeast Florida region.

year ago, I walked out of the doors of Northeast Florida State Hospital, and my life has never been the same since. When I first arrived, I was in one of the darkest places I’ve ever been in my life. I didn’t see much hope, and I didn’t know if I would ever feel better. But during my time there, something changed. Little by little, through the patience, care, and dedication of the staff, I began to heal.The staff there were absolutely incredible. They weren’t just “doing their jobs they were friendly, fair, and went out of their way to make us feel comfortable, cared for, and respected. They created a warm and welcoming environment that felt safe, supportive, and encouraging. The events, the programs, and the way they connected with us made me feel at home, even though I knew it wasn’t my permanent home. They truly made it feel like a family, and that meant the world to me.Since leaving a year ago, my life has become better, stronger, and more powerful than ever before. The strength I found during my time at Northeast Florida State Hospital has carried me forward and helped me keep growing. I often think back to the staff, the experiences, and the kindness I was shown, and I can honestly say I miss them and everything they did to make us feel cared for. I want to say from the bottom of my heart: thank you. Thank you to Northeast Florida State Hospital and every single staff member who went above and beyond to make a difference in my life. Medicaid is a state based program that helps lower-income individuals and families pay for healthcare. Medicaid covers addiction treatment so those enrolled can use their coverage to pay for rehab. When a program accepts Medicaid the client often pays very little or nothing out of their own pocket.

Private insurance refers to any kind of healthcare coverage that isn't from the state or federal government. This includes individual and family plans offered by an employer or purchased from the Insurance Marketplace. Levels of Care

Residential treatment programs are those that offer housing and meals in addition to substance abuse treatment. Rehab facilities that offer residential treatment allow patients to focus solely on recovery, in an environment totally separate from their lives. Some rehab centers specialize in short-term residential treatment (a few days to a week or two), while others solely provide treatment on a long-term basis (several weeks to months). Some offer both, and tailor treatment to the patient's individual requirements.

Rehab centers with dedicated young adult programs will include activities and amenities that cater to this age group, with an emphasis on specialized counseling, peer socialization, and ongoing aftercare.

Clinical Services

Cognitive Behavioral Therapy (CBT) is a therapy modality that focuses on the relationship between one's thoughts, feelings, and behaviors. It is used to establish and allow for healthy responses to thoughts and feelings (instead of unhealthy responses, like using drugs or alcohol). CBT has been proven effective for recovering addicts of all kinds, and is used to strengthen a patient's own self-awareness and ability to self-regulate. CBT allows individuals to monitor their own emotional state, become more adept at communicating with others, and manage stress without needing to engage in substance abuse.

Dialectical Behavior Therapy (DBT) is a modified form of Cognitive Behavioral Therapy (CBT), a treatment designed to help people understand and ultimately affect the relationship between their thoughts, feelings, and behaviors. DBT is often used for individuals who struggle with self-harm behaviors, such as self-mutilation (cutting) and suicidal thoughts, urges, or attempts. It has been proven clinically effective for those who struggle with out-of-control emotions and mental health illnesses like Borderline Personality Disorder.

Experiential therapy is a form of therapy in which clients are encouraged to surface and work through subconscious issues by engaging in real-time experiences. Experiential therapy departs from traditional talk therapy by involving the body, and having clients engage in activities, movements, and physical and emotional expression. This can involve role-play or using props (which can include other people). Experiential therapy can help people process trauma, memories, and emotion quickly, deeply, and in a lasting fashion, leading to substantial and impactful healing.

Group therapy is any therapeutic work that happens in a group (not one-on-one). There are a number of different group therapy modalities, including support groups, experiential therapy, psycho-education, and more. Group therapy involves treatment as well as processing interaction between group members.

In individual therapy, a patient meets one-on-one with a trained psychologist or counselor. Therapy is a pivotal part of effective substance abuse treatment, as it often covers root causes of addiction, including challenges faced by the patient in their social, family, and work/school life.

Nicotine Replacement Therapy (NRT) is a way of getting nicotine into the bloodstream without smoking. It uses products that supply low doses of nicotine to help people stop smoking. The goal of therapy is to cut down on cravings for nicotine and ease the symptoms of nicotine withdrawal.

Nutrition therapy, aka medical nutrition therapy (MNT), is a way of treating physical, emotional, and medical conditions through diet. Specific dietary plans are designed by professional nutritionists or registered dietitians, and patients follow them in order to positively affect their physical and mental health.

Trauma therapy addresses traumatic incidents from a client's past that are likely affecting their present-day experience. Trauma is often one of the primary triggers and potential causes of addiction, and can stem from child sexual abuse, domestic violence, having a parent with a mental illness, losing one or both parents at a young age, teenage or adult sexual assault, or any number of other factors. The purpose of trauma therapy is to allow a patient to process trauma and move through and past it, with the help of trained and compassionate mental health professionals.
