About Terros Health – 27th Avenue Health Center – Primary Care

Terros Health on 27th Avenue is a nonprofit medical facility providing behavioral health and substance abuse treatment. They provide clients of all ages with intensive outpatient, outpatient, and medication assisted treatment (MAT) services. Terros Health also provides integrated primary care services. They are located in Phoenix, Arizona.

The intensive outpatient program (IOP) is a high intensity day treatment program that takes place in groups of 8 to 12 clients. Meetings are available day and night, with clients meeting three times each week for three hour sessions. Services offered include individual and group therapy, dialectical (DBT) and cognitive behavioral therapy (DBT), experiential therapy, life skill development, and an introduction to the 12 Steps program.
The supportive outpatient program is less intensive than IOP and is designed for clients who need relapse support. It features 2 sessions a week. Therapy focuses on life skill development and returning to daily living.
MAT is the use of specific medications alongside traditional therapy to treat opioid addiction. MAT is typically used as part of outpatient treatment.
Terros Health accepts most major insurance plans, which may include ComPsych, Magellan, United Healthcare, Ambetter, and HCSC. Out of network benefits may vary, so clients are advised to confirm coverage with their plan.

These people are clueless when it comes to mental health. They simply want to diagnose you with whatever they can to get you to go in for their treatments. If you're ever scheduled to see a Jasmine Lopez, don't bother, she will diagnose you with a disorder that you don't have and treat you like a child. Highly recommend not going here, and seeing an actual professional that knows what they're talking about.

Accepted Insurance

Terros Health – 27th Avenue Health Center – Primary Care works with several private insurance providers and also accepts private payments when possible, please contact to verify your specific insurance provider.

Other Forms of Payment

Private insurance refers to any kind of healthcare coverage that isn't from the state or federal government. This includes individual and family plans offered by an employer or purchased from the Insurance Marketplace. Every plan will have different requirements and out of pocket costs so be sure to get the full details before you start treatment.

Self-pay involves paying for treatment out of your own pocket. You can use savings or credit, get a personal loan, or receive help from family and friends to fund your treatment. If you don't have insurance or your insurance plan doesn't cover a specific program, self-pay can help ensure you still get the care you need.

Financial aid can take many forms. Centers may have grants or scholarships available to clients who meet eligibility requirements. Programs that receive SAMHSA grants may have financial aid available for those who need treatment as well. Grants and scholarships can help you pai for treatment without having to repay.

Sliding scale payments are based on a client's income and family size. The goal is to make treatment affordable to everyone. By taking these factors into account, addiction recovery care providers help ensure that your treatment does not become a financial burden to you or your family, eliminating one barrier to care.

Medicare is a federal program that provides health insurance for those 65 and older. It also serves people under 65 with chronic and disabling health challenges. To use Medicare for addiction treatment you need to find a program that accepts Medicare and is in network with your plan. Out of pocket costs and preauthorization requirements vary, so always check with your provider.

Military members, veterans, and eligible dependents have access to specific insurance programs that help them get the care they need. TRICARE and VA insurance can help you access low cost or no cost addiction and mental health treatment. Programs that accept military insurance often have targeted treatment focused on the unique challenges military members, veterans, and their families face.

Medicaid is a state based program that helps lower-income individuals and families pay for healthcare. Medicaid covers addiction treatment so those enrolled can use their coverage to pay for rehab. When a program accepts Medicaid the client often pays very little or nothing out of their own pocket.

Addiction Treatments

Levels of Care

outpatient iconOutpatient

Outpatient Programs (OP) are for those seeking mental rehab or drug rehab, but who also stay at home every night. The main difference between outpatient treatment (OP) and intensive outpatient treatment (IOP) lies in the amount of hours the patient spends at the facility. Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

intensive-outpatient iconIntensive Outpatient

Intensive Outpatient programs are for those who want or need a very structured treatment program but who also wish to live at home and continue with certain responsibilities (such as work or school). IOP substance abuse treatment programs vary in duration and intensity, and certain outpatient rehab centers will offer individualized treatment programs. The Intensive Outpatient Program gives patients the chance to participate in a structured treatment plan part-time while they transition back to home and work or other responsibilities.

aftercare iconAftercare Support

Rehab aftercare programs support clients' successful reintegration into their home, workplace, and community. Drug rehab aftercare presumes that recovery requires life-long support. These programs typically offer a wide variety of services customized for the clients' unique and evolving needs. Clients may receive vocational training and career counseling, housing assistance, legal aid, peer coaching, and 12 step program induction, among other services. Clients' case managers often play a lead role in rehab aftercare planning.

12-step icon12-Step

Many addiction recovery centers base their treatment modalities on the 12 step program model, and those in treatment often transition to community-based programs. These programs can also be effective for clients who choose not to enter rehab. Traditionally, 12 step meetings are designed to provide intensive peer support whenever it is needed. In addition to group meetings and peer sponsorship, participants "work the steps" to achieve recovery through a systematic healing of the mind, body, and spirit.

Programs

adult-program thumbnail image

Adult Program

Adult rehab programs include therapies tailored to each client's specific needs, goals, and recovery progress. They are tailored to the specific challenges adult clients may face, including family and work pressures and commitments. From inpatient and residential treatment to various levels of outpatient services, there are many options available. Some facilities also help adults work through co-occurring conditions, like anxiety, that can accompany addiction.
young-adult-program thumbnail image

Young Adult Program

Young adulthood can be an exciting, yet difficult, time of transition. Individuals in their late teens to mid-20s face unique stressors related to school, jobs, families, and social circles, which can lead to a rise in substance use. Rehab centers with dedicated young adult programs will include activities and amenities that cater to this age group, with an emphasis on specialized counseling, peer socialization, and ongoing aftercare.
military-program thumbnail image

Military Program

Serving in the military is both mentally and physically challenging, and can result in trauma that persists even after combat ends. Military programs are tailored to the specific and often complex needs of active duty personnel, veterans, and military families. Clients often access these programs through the U.S. Department of Veterans Affairs (VA).

Some nicotine replacement therapy options are available over the counter. Others require a prescription from your doctor. Common forms in Arizona include nicotine patches, gum, and lozenges. Quit smoking medications are also available.

Called to schedule an appointment and they are talking about there are no appointments available but didn't even put me on hold to check. They have attitudes like answering the phone is inconvenient. If you don't like your job, quit. In this line of work you should be willing and eager to help. People are calling to try and get help. As I asked for an appointment after 3:30 pm and the first Rep said there's absolutely nothing at any of the locations. Checked the website again and see it's open until 8 pm, called back and the second representative had the same weird energy but didn't even check. Just said there's nothing available. So you mean to tell me there are no appointments available at all, after 3:30 pm, in the next 3 months at any location? Terrible customer service.

I was in IOP with Blair Duplantis and she is very dismissive and comes across as rather judgmental. She took me off Webex because I was unable to turn my camera on despite allowing others to leave theirs off in the past, the only reason my camera was off was because my phone was dying which she never had a problem with before, until I called and asked for a new counselor the same day. I called Terros and asked to switch counselors and was not provided with helpful information from the front desk. It feels as though Terros makes it more challenging to get help than it should be. Blair’s intern seemed to actually care about the participants and provide useful perspective more than she did. I have nothing but good things to say about the intern and wish I could remember his name. All I wanted was someone who would work with me but she made it very hard. Especially from someone who should know how to speak to someone who is going through addiction. I found it difficult to communicate with her. She will hang up on you if you speak up.

What a joke of a place. Made an appointment to talk to someone in person weeks ago. I get there, get taken to a room to go talk to an IPAD. Come on, I could have done that from home. If you need help go somewhere else. If I could give minus stars I would.
