UMR is a third party benefits administrator that is part of United Healthcare Services. UMR works with employers who have self funded health plans to provide access to medical provider networks and other benefits strategies.
The company is the nation’s largest third party administrator with over 3,800 benefit plans as clients. Their networks and services help 6 million members access the care they need. Employers can get claims data and reports that help them understand and manage health care costs while also accessing UnitedHealthcare’s large national network.
Does UMR Cover Addiction Treatment?
UMR helps self funded employers build their own unique insurance plans to offer employees. These plans do not have to offer essential benefits as defined by the Affordable Care Act. However if they do offer mental health or addiction treatment coverage it must be at the same level as other medical coverage according to the Mental Health Parity Act.
In order to determine what coverage you have for substance use treatment you will need to talk to your company’s benefit specialist. To maintain your privacy you can ask how to find out what behavioral health providers are in the plan’s network. You can also ask if the plan has any coverage for out of network care.
When you verify insurance coverage before beginning treatment you can ensure you get any referrals or preauthorization that is required. This allows you to avoid any unpleasant surprises while you’re trying to focus on recovery.

This place is a great place to go if you need help with substance abuse or just mental health as well. When I went, I unfortunately didn’t take it very seriously but the people that did take it seriously have been very successful. Kyler was a great mentor while I was there, and he along with the other techs all seem very genuine and honest and willing to help in anyway they can. I am extremely grateful for this place, as well as Kyler especially. I don’t think I would be where I am today without him as a mentor.
I only have one small complaint: I craved a latte every day but only managed to snag one on a single occasion. Other than that, they really take care of you. The place is fantastic, and the staff, including April, is super friendly. Definitely recommend!

I initially planned to stay at the Ohana addiction treatment center for just two weeks, with the idea of checking in on how I was feeling during my time there. But honestly, from the fantastic staff to the cozy accommodations, it felt just like home. This place is really ideal for anyone looking to stay sober.

Zinnia overall was a great experience! I encountered many strengths and a few faults. The faults came under scrutiny in the upper management role. I completely understand that Zinnia is a wellness facility and a business. The personal experience, to me was wellness was priority one, in the case of facilitators, techs and nurses. This was not the case in the roll of Director of Operations at the Singer Island facility. This individual showed little compassion and it was obvious that business outranked wellness.Regards,Bob S. Prosperity Haven/ Wellbrook Recovery is such a wonderful place that helped me get sober. From the moment you walk in, everyone is so warm and welcome. When looking for a facility, WRC covered all the bases with being male only with fewer patients than a traditional set-up.The group instructors are diverse and will push you to be vulnerable and work outside of your comfort zone. There are all different types of therapies that are used to help you heal. The entire place/ program is about fellowship. Seeing the alumni come through the doors and support the guys in the house was huge for me.10/10 would recommend this facility to anyone seeking sobriety!

I worked at Prosperity Haven for two years. Working here was a truly transformative experience, both personally and professionally. The leadership at Prosperity Haven was exceptional supportive, open to feedback, and always striving to provide the best care possible. The culture of compassion and respect that permeates the facility is remarkable, and it s evident (in everything they do) that the staff is fully committed to the mission of helping individuals heal and thrive. If you're looking for a place where you can start or pick up your recovery journey in a supportive and compassionate environment, I cannot recommend Prosperity Haven Treatment Center highly enough. The success stories I got to witness were genuinely life changing and my time there was an experience I will always cherish.

Lifeskills is a program I didn t want to attend at first, but now after completing my time here I m so happy I did! I was able to take the time I needed to focus on bettering myself which is something I am eternally grateful for. I couldn t have done this without the support of all the therapists, doctors, techs and other clients. Dr. Nicolle, Dr. Spaedy and Ilissa were a huge aid in my mental health journey and recovery. I owe a lot of my success to them but also myself. They taught me I can do a lot more than I thought and how to believe in myself again! Several techs including Robince, Amber, Holly, Tamara, Garfield, and Sabastian played a huge factor in the recovery process outside of the classroom. Their positive attitudes helped me through the ups and downs of what recovery can be. They were a crucial part of the process and I will be forever grateful to everyone who helped me along the way. Thank you Lifeskills for helping me back on my feet and allowing me to blossom into the person I always wanted to be.

Upon my arrival at Lifeskills, I was a lost alcoholic who self-medicated with substances and I had lost my purpose in each and every way. I felt betrayed by those close to me, when first surprisingly 5150d and second shockingly Marchman Acted; thus, my arrival, in my eyes, was less than amicable and left a lot of confusion, to say the least.Arriving in FTL, undergoing a detox protocol, I received superb one on one attention, which heightened when I ended up in a wheelchair due to my severe withdrawals. Upon completing detox at FTL, I was transferred to Deerfield Beach and instantaneously felt a breath of fresh air. Swiftly, I was taken under the care of Tara, my primary therapist, Dr. Mirella, my family therapist, Dr. Spaedy, my psychiatrist; along with Dani, the Alumni Coordinator, and techs Gus, Kevin, Garfield and Diamond, who all were indispensable to me. I am so grateful to them for navigating my way back to finding candid reasons to not just survive but also lighting a fire for me to find purpose n' thrive.Due to the spectacular care at Lifeskills Deerfield Beach, which will be 99 days in totality, I was able to find the real me, be confident in my ability to stay sober, and discover my new life, which months ago I never thought was possible. I will always be very grateful to my whole clinical team, who I have to give a sincere shoutout to Dr. Forstadt, who ran not only my favorite group, Male Trauma, but also conducted the in-depth neuropsych testing.I hope that I can be an anchor for someone experiencing the alcoholic / drug addicted tailspin, in that Lifeskills South Florida gave me the key to escape.

I've spent the last 4 years of my life going through different treatment centers, all of them claiming individualized treatment, yet lifeskills is the first one I ve actually gotten an individualized treatment. My work with Doctor Alex has been wonderful and I ve gained more out of my time with her than I have with any other therapist ever. Dani has been great as well and has gotten me ready for my transition into the less intensive php at Lifeskills. So grateful for staff like Kevin, Garfield, Stephen. As every little part of this program contributes to the wonderful experience I ve had here. Thanks lifeskills
My family's experience with Avenues Recovery Center was truly life-changing. My sister struggled with severe drug addiction for years, and we felt lost and hopeless. Reaching out to Avenues was one of the best decisions we ever made. From the moment we contacted them, the staff was compassionate, understanding, and professional. They provided a safe and supportive environment for my sister to begin her recovery journey. The treatment program was comprehensive and tailored to her individual needs. The therapists and counselors were incredibly skilled and dedicated, helping her address the root causes of her addiction and develop healthy coping mechanisms. The aftercare support was also invaluable, providing her with the tools and resources she needed to maintain her sobriety.Today, my sister is four+ years sober and now an employee at Avenues Recovery Center, helping others on their recovery journeys. She's happily married and just welcomed her second child. We are incredibly grateful to Avenues Recovery Center for giving her, and our family, a second chance at life. We can't recommend them highly enough. If you or a loved one is struggling with addiction, please reach out to them. They offer hope and healing, and they truly care. Thank you, Avenues!

Oh and you eat like a king lol some of the best food ever and the bakery is out of this world. In all seriousness this place is an amazing place to get help. I felt welcomed by all the other guests as well. Everyone is there for the same reason so its very easy to get along and you build a family forever . I've been out for over 6 months now and I still talk to at least 15 of the guys. Highly recommend. In fact I have already recommend it to a few people. I am a two time alum of High Watch and I owe the place so much. The first time I went in July 2023 was the first time I ever tried to get sober - I was terrified to enter treatment, didn t want the help, was angry and frankly wasn t ready to get sober. High Watch nonetheless showed me so much love, support and encouragement during my 28 day stay, and in the months following my discharge. They met me where I was and didn t give up trying to help me. This stay at High Watch was the first time in my life I felt like I belonged somewhere.I came back 6 months later in February 2024 and this time stayed for 4.5 months - completed the regular 28 day program and also the 3 month extended guest program. I am still sober today. If it hadn t been for my extremely positive experience during round 1, I would never have returned. The staff at High Watch genuinely care about every single guest there, the therapists and group sessions are great, and the campus is beautiful. There are quite a lot of rules / a rigid daily schedule, which was sometimes tough but for me I needed this kind of structure. What s also cool is that so many (I think the majority) of staff members are in recovery themselves and many have gone through High Watch so they get what it s like to be in treatment and how hard it is, especially at the beginning. High Watch also helps set you up with plans after your discharge and have great connections with sober livings and IOPs to help with your aftercare plans. For what it s worth, the food is also amazing

The facility is beautiful and maintained very well. I came here feeling like I had changed into a person I wasn t proud of after experiencing intense traumas throughout life, I am leaving feeling lighter and brighter and much more like the best parts of myself. The chefs are great and accommodating at all times, my therapist Kevin helped me through things I didn t think were things I could work through. The techs Chay, Natalie, Chelsea, and Kailea are full of kindness and will make you feel at home and secure. On the negative side I do think the program lacks consistent communication at times which can be triggering, to be told one thing and it ends up being different.

I went to Wolf Creek about 2 years ago. I struggled the whole time I was there but it wasn't really their fault. Some rehabs like to keep you occupied/busy so you can keep your mind off of your life outside of rehab. Shocking your routine is a must. Wolf Creek does do that for sure. They like to keep you on a tight schedule. For the most part rehab to me is just the same just about anywhere. You know what the answer is already but you're just not doing it (for me anyways). It takes work. That work may not always look the same to everybody else. I like the counselors I had. I like the house managers. Some of the things I would have liked to have seen done more was management within the home. Their idea was for us to keep each other accountable which can be quite stressful when you're trying to get better. The one thing about any rehab that I've never really quite understood is when there is people there that are obviously not trying to get better and they still keep them. That's going to effect every single person in treatment. Possibly cause others around them to relapse. (I get some cases are harder then other but damn)Maybe a little bit more than $75 a week from Fry's would have nice as well. It was quite the struggle for some of the people that didn't have any extra money. So us other patients/clients would pick up the slack.Remember this was 2 years ago. Rehab is what you make it. You can choose to take those obstacles that you see everyday while you're in treatment and be negative about it or you can see it as practice. Those times where you don't get the Monster Energy drink that you want that morning. Sit with it. Realize it's not the end of the world. You're there to get better, not get pampered. Put in the work and be nice to the people there. All rehabs are businesses but the people make the place and the people there are awesome love you guys. Some tips for going--Get an mp3 player that does not have wifi access ( music saved me )- get it before you go and upload music to it.- FMLA and stuff like that is much easier to do and start before you leave for Rehab. Trust me on this. Check with your HR department at your job to make sure you know how to file FMLA properly. Some companies will give you short-term disability but they require a lot of stupid little loopholes you have to go through. That will not be wolf Creek's fault just remember that.- if you have meds already. Try to get all doctors information beforehand it will make things much quicker for everybody else in the officeTrust me I know it's hard to think of any of this stuff when you're in the dark but you got this

What Products Does UMR Offer to Employers?
UMR offers a wide variety of products and services to employers including backend solutions for benefits coordination and payment integrity.
The most important product is the connection to a high quality medical provider network. Because they are a subsidiary of United Healthcare, clients have access to high quality and cost effective providers. They also have cost management solutions and reports that help employers fully understand the use of benefits.
For member service there are three options. The Customer First model allows employees who are insurance plan members to contact a designated customer care team that’s trained on the employer’s specific plan and benefits. The Plan Advisor model provides advocacy service so members can have help navigating benefits with personalized care. There’s a single contact for members to ask questions and get information about finding the best care for their needs as well as a nurse helpline.
The GenerationYouTM service model gives the highest level of support which includes advocacy and engagement to promote better health outcomes. If your employer has GenerationYouTM you will have digital access that gives you information about your benefits and providers along with highly personalized help accessing healthcare.
UMR also provides help for employers who want to offer financial accounts like FSAs or HSAs. They can also administer HRAs and lifestyle accounts (LSAs) along with Retiree reimbursement accounts (RRAs).
As an employee member you may also have access to case management for complex health conditions and programs that provide incentives for wellness and healthy lifestyle choices.
What Inpatient Addiction Treatment Does UMR Cover?
Self funded plans are not legally mandated to cover addiction treatment. However, many will offer these benefits. As the central focus of insurance is on providing medically necessary care you will always need get a treatment plan from your doctor before starting any intervention — like inpatient addiction treatment.
The American Society of Addiction Medicine (ASAM) Criteria is an assessment framework medical professionals use to determine appropriate care levels. With this tool—which evaluates criteria like severe withdrawal risk and underlying medical conditions—a medical professional will assess your need for residential care.
If inpatient treatment is the best route for you then medical detox is usually the first step. During this phase individuals are under round the clock supervision to manage withdrawal symptoms safely after stopping the substance of abuse.
Inpatient or residential care is typically the next stage in a person’s recovery journey. These settings provide a structured environment created with one primary purpose: assisting individuals to live a drug free life. Residential care benefits individuals in recovery by providing targeted treatment and reducing relapse risks while also offering continuous support.
Recovery promoting activities like individual and group therapy along with medication management and life skills workshops characterize a typical day in an inpatient program. The goal is to treat the whole person which means addressing the physical and psychological as well as social aspects of addiction and equipping individuals with the tools needed for long term recovery.
What Outpatient Rehab Does UMR Cover?
Your employer’s insurance plan may partner with UMR to cover three outpatient rehab approaches: Partial Hospitalization Programs (PHP) and Intensive Outpatient Programs (IOP) along with general outpatient care.
A PHP is the more involved alternative as it can include 20+ hours of weekly treatment. You attend during the day and return home at night. An IOP requires 3 to 5 days of weekly programming while general outpatient care—the most flexible mode—offers sessions that vary in frequency based on each person’s case.
Outpatient programs are structured to assist individuals in rejoining their social and professional lives in the most useful way. Individual and group counseling along with skills training are some methodologies employed to help you develop new habits and coping strategies while maintaining your daily responsibilities. Aftercare services follow outpatient treatment to strengthen a person’s support networks and sustain long-term recovery.
Always verify your insurance coverage before committing to a treatment plan as self funded plans can differ widely in terms of coverage and limits along with requirements for outpatient rehab services.
Does UMR Cover Methadone & Suboxone (MAT)?
If your insurance plan covers MAT this approach can be a helpful tool for treating opioid and alcohol abuse disorders.
In the initial stage of treating opioid addiction medications like methadone or buprenorphine are critical during the medical detox step—when the drug gets out of a person’s body—to manage withdrawal symptoms.
For a long term opioid free life MAT can help by incorporating medication such as naltrexone and Suboxone which help normalize brain chemistry—disrupted by substance abuse—and control cravings that can persist for months. This allows you to stay stable in treatment and lowers the chance of treatment.
MAT can be a valuable option for treating alcohol addiction. Medical professionals employ medications in the initial detox phase to cope with the withdrawal symptoms of stopping alcohol consumption. In the later stages of the recovery journey medications like naltrexone and acamprosate can reduce alcohol cravings and support long term sobriety.
MAT interventions are most effective when combined with counseling to empower individuals with life skills to establish long term recovery and sustain treatment outcomes. Over time you can taper off the medication support and live fully substance free.
Remember to verify coverage before committing to a plan. You may need a referral or preauthorization as plans can differ regarding individual medications and coverage requirements.
Does UMR Cover Other Mental Healthcare?
Self funded insurance plans are not legally ordered to cover mental health but many do. Always check your insurance coverage before initiating care to prevent unanticipated costs.
Mental healthcare demands an individualized approach. Some individuals struggle with mental health issues without other disorders. In these circumstances treatment will generally start with therapy sessions with qualified professionals. These are used to identify the appropriate care you need which might include counseling along with group sessions and potentially medication.
Individuals who struggle with addiction commonly have co-occurring mental health disorders as well. About 21.5 million adults in the United States battle addiction while facing mental health challenges like depression and anxiety along with mood disorders.
In these cases treatment must address substance abuse and co occurring conditions simultaneously to increase the chances of success on both fronts. Why? Because mental health issues make individuals more vulnerable to drug abuse. Also medications for mental health disorders like anxiety can interact with substance use so a holistic approach that considers every aspect of a person’s mental health and circumstances along with underlying medical conditions and crisis support is vital to obtain long term favorable outcomes.
Does Addiction Treatment Need to Be In Network?
Self funded plan coverages vary much more than other insurance plans. However, always keep in mind that self funded plans are focused on cost effective care. It’s possible that some of these plans may cover out of network care but not all plans will. If out of network care is covered it will be at a higher cost for you. For example you’ll usually have much higher deductibles and copayments.
Before initiating treatment double check network status and coverage details with your insurance plan and UMR. In network providers are your best choice to keep out of pocket costs low and maximize your insurance’s benefits. Out of network care can significantly increase your expenses so it’s best to utilize the network to your advantage.
What to Do if UMR Won’t Cover the Cost of Rehab
Self funded plans may cover addiction treatment options. However they may be more limited than traditional plans and you may encounter less comprehensive benefits. Don’t let this fact discourage you from seeking the care you need.
Consider contacting treatment centers and ask if they have sliding scales of fees based on income or offer scholarships or payment plans to facilitate payment.
Get resourceful and seek free and low cost rehabs such as the Salvation Army—which permits you to work instead of paying—or a county or state run program.
Addiction affects every aspect of your life so make treatment a priority. Borrow money or rely on your savings to advance on your path to recovery; the effort may change your life.
How to Verify Coverage With UMR
The best way to verify coverage with your insurance plan and UMR is to reach out to your organization’s benefits manager or contact person. If you have been provided a customer service number take advantage of it to access reliable coverage information.
If your employer suggests you contact UMR directly you can rely on their excellent customer service to help you determine what treatment is best and where to get it based on your plan.
Refrain from contacting treatment centers directly for insurance verification because each self funded insurance plan is unique. Treatment centers may accidentally provide you with wrong information that doesn’t relate to your real benefits.
Calls to any general helpline will be answered or returned by a paid advertiser.
By calling the helpline you agree to the terms of use. We do not receive any commission or fee that is dependent upon which treatment provider a caller chooses. There is no obligation to enter treatment.
