Providence St. Joseph Health was formed in July 2016 when two nonprofit Catholic healthcare organizations–Providence Health & Services and St. Joseph Health–merged to create a new company.
This company became known as Providence St. Joseph Health. In 2019 the company rebranded under the Providence Health & Services name although St. Jospeh’s is still a hospital within their system.
The different providers within the Providence St. Joseph Health family operate as a healthcare collective which means that they utilize their own network of providers and partners to serve their patients.
The insurance arm of Providence St. Joseph Health–Providence Health Plan–operates mostly in Oregon and southwest Washington but they also collaborate with network partners in other states such as California. In 2000 Providence Health Plan provided insurance for 326,000 members. In 2024 they insure about 375,000 members.
Does Providence Health Plan Cover Addiction Treatment?
Maintaining compliance with the 2010 Affordable Care Act (ACA) most Providence Health insurance plans do offer mental health and substance abuse treatment as well as other medical care. The ACA defined 10 essential health benefits that insurance plans need to cover and the list includes mental health and substance use treatment.
However there are exceptions to the law so it’s very important to verify your insurance details before you commit to treatment. You may also need to get a referral or preauthorization before rehab is covered. Work closely with your doctor to set up a treatment plan that fits your needs so you know what kind of addiction treatment to look for.
Celebrities in recovery lol love this place so much. They helped me a lot with getting going in the right direction and what to pursue out of life. I was able to surrender my past and move on from my old ways through their programming.

Scottsdale Recovery has changed my life. If you or someone you know needs detox and/or treatment. This is the place. The staff is strategically handpicked by professionals; peers who have been through addiction and have come out the other end. The resources and services they provide are phenomenal, I would highly recommend SRC a million times over.
From day 1 to graduation, they guided me through, to a complete life changing transformation.

Thee best experience in recovery I've ever had. If anyone needs help with drug or alcohol addiction please call Serenity Springs. Their staff is amazing.
After years of battling alcoholism, I finally surrendered and flew out to LA from St. Louis, MO to get into recovery. I entered the detox facility on 5/21/2025, graduated to the sober living facility on 6/22/2025 and graduated the IBH program today on 9/18/2025. I had been wanting to live a sober life for years, but I never had the strength to do it by myself. IBH gave me that strength. I learned a lot about my disease here, a lot about what makes me “me,” and I learned how to not only live sober, but also live happy. They say the disease of addiction is a life or death situation. This program helped me save my own life. The friends I’ve made along the way, the community I was immersed in, and how welcoming the staff made me feel has helped me build a strong, healthy foundation for life long sobriety. I’m 120 days sober, and I’m happy to say that I’m part of the IBH family.

This experience has been a real eye opener to me was able to love and be proud of myself after 22 years and found the lord my god after 25 years of turning my back on him. Yes there was ups and downs the staff needs to work on communication but we all need to work on this. But the group of people and helping me begin this wonderful journey to the rest of my life has been amazing.
I had a great case manager (James Stevens) he helped me with everything I needed and supported my after care. Thank you James !!

If you need anything to figure out in your steps of healing and making a journey of rediscovering yourself.
Royal life treatment center is a place to start with.
Respects to all the staffs at the centers
Who give their full attention and actually hear you out. Shout out to my Case Manager Tristan!
Intake person told me “May 24th is a great day to be sober. Has a good ring to it.” That alone kept me sober. 748 days later, I am still sober. Thank you Hazelden!! It’s the little things.

Such a transformative experience! Naturally, the reviews you read for addiction treatment centers are going to skew negative but don't let that deter you if you believe you need help. Not only did I have an amazing experience with staff, I've built a strong foundation of community among my peers who were also seeking treatment. While encouraged by their use of an evidenced-based model, the incorporation of the 12-steps had me skeptical at first but now I'm fully committed. If you feel the 12 steps aren't for you, they also provide information about alternatives such as SMART Recovery and Recovery Dharma. My one complaint would be that I had chosen to participate in the in-person IOP during intake, but was somehow placed into the virtual IOP. Once it was recognized it was a quick fix and it didn't impact my start date or anything like that.

Could not have been more pleased with my experience and have grown so much in my 10 months of outpatient treatment. Cleidy and Mark will be in my recovery forever and cannot thank enough.
Nsight saved my life…I started with them a year ago in their Residential program and worked my way down to outpatient because I didn’t want to leave my amazing individual therapist (Alex) along with the whole clinical team who had my back through my darkest times. I cannot thank Nsight enough for everything they have done- you can clearly tell the team/staff was there for the well being of the clients, not a paycheck (theyre the most caring and genuine people ever!). I love Nsight, from the bottom of my heart and I owe them my life<3

I'm embarrassed to say I had been to a few treatment centers before I was fortunate to be placed at Nsight, although I know that's true for many people who struggle with their mental health and addiction. If you experienced trauma during childhood or at any point in your life and it affects your mental health, or you're not even sure if it does but you struggle with addiction, Nsight is the place for you. There is no other treatment center like it. Their program, groups, counselors and housing are one of a kind. The environment is kept professional unlike many other facilities. The counselors are actually educated and experienced as well as empathetic and relationship based. Nsight is so different in so many ways from other treatment centers. Nsight saved and forever changed my life. It is through Nsight that I found myself. I graduated and walked out of their program a literally completely different and better person. I went home and my mom cried and said she had no idea this is who I am and that I am actually such a beautiful, kind and fun person. After struggling with depression, anxiety, addiction and other diagnosis that were inaccurate. I owe my life today to Nsight, their program and their counselors. I cannot recommend this program enough. If you're on the fence between choosing Nsight and another place - choose Nsight. You won't regret it.
The staff is the best! They genuinely want to see you grow and do better!

Honest rating of overall facility.
This place was genuinely a life changer for me, and I could see that in other kids there as well. I loved almost every therapist there, and most of the kids were friendly. I really liked the adolescent room because it was spacious, and it was always clean. The bathrooms never had any paper towels, but other than that the restrooms were very clean. Honestly, I can't ever remember a time that the facility was dirty or cramped.
Honest rating on each therapist.
Ashley - Ashley is awesome! During my time at Embark, Ashley ran most of our groups. She was so kind, and compassionate. I feel like she genuinely perused this kind of work because it's her passion to help people. She always listened and made sure everybody got a chance to talk. She also is very observant, and checks in if you're not having the best day. Which is really nice sometimes to know somebody notices and cares is comforting.
David - David is also a really cool guy. He ran group often. He can be a little bit rough sometimes, but everybody has different ideas on how to do things. For example, if you were sleeping during therapy, he'd bang on the table, which can be a trigger for some people. He can also be a little bit strict sometimes, for example taking the markers, or popsicle sticks that we're aloud to have with every other therapist. From what I heard he does really well as an individual, or family therapist. Overall, I think he genuinely is passionate about his job, is a good person, and cares a lot about the kids even if it might not seem it sometimes.
Elvin - Awesome dude! Elvin was genuinely so kind and always energetic. I think he wrote prescription medicine, and if you have any problems with that, he gets it resolved very fast. He only did group sometimes for adolescents when I was there, but when he did an amazing job in my opinion. He's genuinely such a cool dude he will talk to you about literally anything, he's an open book and has some pretty cool stories!
Myron - Myron was my individual and family therapist. Genuinely such a cool, funny, nice dude! I really enjoyed my individual therapy sessions with him, and he was the first therapists I felt I ever made progress with. He didn't do group much, so I can't really say if he's good at it, but I'm sure he is! He really cares about his patients.
Jamonica - Jamonica was someone who also did groups quite often. Her energy is so great, and she was soooo kind to me while I was there. She's such a sweet person, and you can really tell how passionate about her job she is. I've also heard she's an amazing individual and family therapist.
Lauren - I rarely ever talked to her. She rarely did group for the adolescents, but some of the other adolescents said she's an amazing individual therapist. I didn't not talk to her for any specific reason; she just didn't do group much and primarily was in her office. I talked to her maybe once or twice, and when I did, she was nice.
Maya - She ran group occasionally and was perfectly fine. She wasn't my personal favorite therapist, but not for any bad reason. I didn't really love the skills she taught, and how she taught them, but nothing against her at all. I'm not sure if she's an individual therapist.
Med management (Dr. Hooper) - Dr. Hooper is super friendly, and really good at finding the right medication fits for you. Php meets with her once every week, while Iop meets with her once every 2 weeks.
Maria - Maria is also a nice lady. I'm honestly not sure what she does, but when I was enrolling, she did the paperwork and stuff. You will also take outcomes, which ask you questions about how you're dealing with anxiety, depression, etc. She has access to those outcomes, so 100% feel free to ask her to take a look at them!
Overall, I think the staff is awesome!
Please check bags better, Embark. The bag checking is honestly a joke. There are people who deal with all kinds of things, and not saying anything would happen but please check better just in case.
Goodbye, Team Embark, I'll miss you guys!

We have been on a mental health journey with our daughter for quite some time. Nothing ever seemed to click, and past PHP programs caused only more harm and trauma with no healing. Since joining the Embark program we can see glimmers of it working. I know that she feels safe here, and has confidence in her care (as do we). We attended a family night and when they say family they clearly include the caregivers and staff along with the patients and their family. I say this because the whole vibe was joy and love. I couldn't be happier (or more relieved) about our choice.
So far I spoke to Dr.today about my issues I'm having and I just talk to Brittany and the both of them was very helpful and Professional.so far so good and 👍 for you have a blessed see y'all tomorrow thx again MJ

During my month in their inpatient treatment program, I was surprised by how supported I felt. The counselors genuinely listened and stayed focused, and for the first time in a long time, I left with a clear plan and renewed hope after feeling completely lost.

Frequently Asked Questions
Where Does Providence Health Plan Operate?
Providence Health Plan utilizes the Providence St. Joseph Health network of more than one million hospitals and doctors along with mental health providers and pharmacies to deliver care. There are three networks: Providence Connect and Providence Choice as well as Providence Signature.
Insured members have a “medical home” which includes access to their primary care provider at a clinic or hospital. Providence Connect and Providence Choice are available in specific counties within Oregon. The ProvidenceSignature network is a nationwide provider network.
Some services are available in California and Montana along with Texas and New Mexico through collaborating providers. They also have collaborating providers in Alaska.
Types of Insurance Plans Offered by Providence Health Plan
Providence Health Plan offers individual and family plans in Oregon and Washington. They also have insurance plans that employers can use to provide coverage to employees and they have Medicare and Medicaid plans.
Individual and family plans are available directly from the company and on the Insurance Marketplace. They offer all the benefits required by the ACA including mental health and addiction treatment coverage. Many of the plans will pay a percentage of the cost of inpatient and intensive rehab — such as 70% — and then you pay the remainder. The individual and family plans require you to use in network providers unless there’s an emergency or need for urgent care.
Providence Health’s Medicaid plan is called the “Providence Orgeon Standard Plan.” It’s available for lower income individuals and families that qualify for Oregon Medicaid. This plan also covers addiction treatment and mental health and you may be able to get free or low cost care.
Finally Providence Health Plan has two different Medicare plan options. If you have Original Medicare and need help covering your out of pocket costs you can get a Medicare Supplement plans from Providence. Providence also offers Medicare Advantage plans which replace Original Medicare and often have additional benefits such as vision and dental coverage. However you will need to use the Advantage plan’s provider network to receive coverage.
What Inpatient Rehab Does Providence Health Plan Cover?
Your insurance plan will focus on covering medically necessary care. Not everyone needs inpatient or residential rehab but if you do most Providence Health plans will cover at least part of the cost. Addiction treatment is covered as a behavioral health service.
Many people will start with medical detox. This process allows substances to leave your body while you are monitored by doctors and nurses who can help treat withdrawal symptoms. Medication assisted treatment (MAT) is commonly part of medical detox. MAT uses FDA approved medication to manage withdrawal symptoms so that detox is safe and as comfortable as possible.
Inpatient treatment takes place in a clinic or hospital and residential treatment takes place in a facility with a more home like setting. Both provide 24/7 supervision and care.
Inpatient treatment is often for more acute needs and typically lasts one or two weeks. Residential treatment is longer and often lasts 28 to 30 days or more. People experiencing both addiction and a mental health diagnosis—which is known as a dual diagnosis—often require inpatient treatment.
During inpatient or residential treatment you’ll get individual and group therapy along with recreation options. Many programs also offer holistic treatment options like art and music therapy along with yoga and meditation.
What Outpatient Addiction Treatment Does Providence Health Cover?
Most Providence Health Plan insurance covers a variety of outpatient addiction treatment services, including partial hospitalization programs (PHP) and intensive outpatient programs (IOP). Its Charlie Health program is a virtual intensive outpatient program (vIOP) designed specifically for teens and young adults, ages 11 – 30, who are struggling with addiction and other mental health issues.
There is also an outpatient substance use disorder treatment program for adults age 18 and older available through Providence St. Vincent Outpatient Behavioral Health. This 90-day program includes assessment, personalized treatment planning, group therapy, and individual therapy sessions (both in-person and virtual).
Does Providence Health Plans Cover Methadone & Suboxone (MAT)?
Yes, most Providence Health Plans coverage does include MAT programs if it’s medically necessary for you, including Methadone and Suboxone. However, checking your coverage to get the full details and see whether you need prior authorization is always wise.
Methadone and Suboxone are MAT treatments used to treat opioid addiction and help alleviate withdrawal symptoms and cravings. The treatment programs usually involve regular visits for medication management and weekly or bi-weekly therapy visits. One MAT program available in the Providence Health Plan network is at St. Joseph Hospital in Orange, California.
Does Providence Health Plans Cover Other Mental Healthcare?
Providence Health Plans coverage generally offers a variety of behavioral health services and programs to meet your personal needs. This may include inpatient, outpatient, and substance use treatment programs. Also Providence Health Plans provides care management and crisis support, self-management and mindfulness tools, and other resources for improving overall well-being.
The insurer’s Behavioral Health Concierge offers direct access to Providence Health providers who can help with mental health and addiction issues and co-occurring disorders. This service is available to eligible Providence Health Plan members who live in Oregon, Washington, Idaho, California, Montana, and Texas.
As with all insurance plans, coverage will vary so be sure to get the full details on your plan before you start treatment.
