Clients in intensive outpatient programs (IOP) receive robust, personalized care to support their reintegration into their community. Clients stepping down from inpatient treatment frequently enroll in IOP before entering standard outpatient care or community-based recovery programs, such as AA. Intensive outpatient treatment generally involves between nine and 20 therapeutic hours weekly, with the frequency and duration of sessions decreasing as clients stabilize. Clinical Services

Cognitive Behavioral Therapy (CBT) is a therapy modality that focuses on the relationship between one's thoughts, feelings, and behaviors. It is used to establish and allow for healthy responses to thoughts and feelings (instead of unhealthy responses, like using drugs or alcohol). CBT has been proven effective for recovering addicts of all kinds, and is used to strengthen a patient's own self-awareness and ability to self-regulate. CBT allows individuals to monitor their own emotional state, become more adept at communicating with others, and manage stress without needing to engage in substance abuse.

Whether a marriage or other committed relationship, an intimate partnership is one of the most important aspects of a person's life. Destructive behaviors like addiction, eating disorders and sex addiction affects both members of a couple in deep and meaningful ways, as does rehab and recovery. Couples therapy teaches partners to build healthy patterns to support recovery.

Creativity is inherently healing, and can help those in recovery express thoughts or feelings they might not otherwise be able to. The Ranch TN offers creative ways for clients to express themselves such as art and music.

Dialectical Behavior Therapy (DBT) is a modified form of Cognitive Behavioral Therapy (CBT), a treatment designed to help people understand and ultimately affect the relationship between their thoughts, feelings, and behaviors. DBT is often used for individuals who struggle with self-harm behaviors, such as self-mutilation (cutting) and suicidal thoughts, urges, or attempts. It has been proven clinically effective for those who struggle with out-of-control emotions and mental health illnesses like Borderline Personality Disorder.

The Ranch treats women with anorexia, bulimia, binge eating, and other disordered eating patterns. Clients learn the underlying reasons behind their destructive eating patterns. They learn to look at food as a source of nourishment and not a way to cope with emotional pain. Body image work, trauma work and equine therapy are main components of the program.

Equine therapy involves caring for and interacting with horses in ways that provide valuable lessons in accountability and in how we relate to others. Equine therapy is an effective approach for addressing and processing underlying issues stemming from trauma and negative past experiences. The Ranch equine program includes equine-assisted therapy groups as well as opportunities for therapeutic and recreational riding.

Experiential therapy is a form of therapy in which clients are encouraged to surface and work through subconscious issues by engaging in real-time experiences. Experiential therapy departs from traditional talk therapy by involving the body, and having clients engage in activities, movements, and physical and emotional expression. This can involve role-play or using props (which can include other people). Experiential therapy can help people process trauma, memories, and emotion quickly, deeply, and in a lasting fashion, leading to substantial and impactful healing.

Eye Movement Desensitization and Reprocessing is based on the discovery that rapid movements of the eye while recalling certain events can reduce anxiety associated with trauma. It has been found effective in clinical trials and has been approved by the American Psychiatric Association, the American Psychological Association and the Veteran’s Administration.

The Ranch works individually with both clients and their families to address the issues that have led the client into treatment. They take a family systems approach to treatment, recognizing the family as a connected, living system; an issue with one of its members is indicative of issues that need to be addressed as a whole unit. They help families understand the various roles each of its members plays in the system, and facilitate healing of the whole unit and the individual parts.

Clients share with others who understand what they're going through. A mental health professional guides discussions and experiential activities. Clients gain new insights into themselves and their relationships.

Clients work closely with a trained mental health professional to get to the root causes of their destructive behaviors. These often include emotional pain, trauma, difficult relationships, attachment issues, low self-esteem and mental health disorders.

Life skills trainings involve all the skills a person must have in order to function successfully in the world. These include time management, career guidance, money management, and effective communication.

Motivational Interviewing (MI) is a clinical approach to help people shift behavior in positive ways. It is more goal-oriented than traditional psychotherapy, as MI counselors directly attempt to get clients to consider making behavioral change (rather than wait for them to come to conclusions themselves). Its primary purpose is to resolve ambivalence and help clients become able to make healthy choices freely.

Recreational therapy (aka therapeutic recreation) uses creative and fun activities to help with recovery. Recreational therapists lead clients in entertaining and engaging activities like sports or games; art (drawing, painting, sculpture); drama, music, and dance; and/or community outings (field trips) to improve patients' physical, social, and emotional well-being.

Trauma therapy addresses traumatic incidents from a client's past that are likely affecting their present-day experience. Trauma is often one of the primary triggers and potential causes of destructive behaviors, and can stem from child sexual abuse, domestic violence, having a parent with a mental illness, losing one or both parents at a young age, teenage or adult sexual assault, or any number of other factors. The purpose of trauma therapy is to allow clients to process trauma and loosen the grip it has on their present day life.

Many individuals who are experiencing addiction are extremely malnourished. This is due to negative patterns that develop during active addiction. Nutrition therapy helps break these negative patterns and teaches you to develop healthy ones that will help you sustain sobriety long term.

My husband and I participated in the family program at The Ranch and I would highly recommend participating in it. We learned so much about boundaries, codependency, attachment styles, communication and even more. The facilitator, Charlie, is super knowledgeable in addiction recovery and the program was extremely beneficial!

I’m writing this as the former spouse of someone who attended The Ranch Tennessee for compulsive sexual behavior and pornography addiction. I supported him going because I hoped it would lead to honesty, accountability, and healing after years of emotional and financial abuse, coercive control, and betrayal in our marriage. Unfortunately, the way the family therapy was handled caused even more harm. I was invited to only one family therapy session. During that call, my former spouse denied cheating and minimized what happened, even though I knew he had been lying to me for years and had secretly spent tens of thousands of dollars on pornography. The therapist did not question his denial or ask for accountability. Instead, she shifted the focus entirely onto me. I was asked about my childhood and was made to feel like I was the problem — “controlling” or “part of the issue” — simply because I was hurt, asking for honesty, and questioning why he showed no remorse or willingness to repair anything. There was no acknowledgment of the emotional abuse, financial harm, or coercive control I experienced throughout the marriage. I wasn’t treated like someone impacted by betrayal — I was treated like I was causing it. It felt like they were using an outdated model where spouses of addicts are labeled as codependent or controlling, rather than recognizing betrayal trauma — a very real and documented trauma response to deception, financial betrayal, and emotional abuse. Instead of addressing the harm he caused, the therapist focused on analyzing me. After that session, I reached out to ask when he would be discharged and whether longer treatment was being considered, because he showed no remorse and was acting like nothing was serious. A few days later, I was told he revoked my access to any information, and the facility would not even tell me when he would be released. When he came home, he said treatment “felt like a vacation.” There was no accountability, no change, no empathy. He also told me his therapist said I was controlling and that he should cut off all contact with me. I’m posting this so other spouses and family members know to ask specific questions before getting involved in family therapy here: - Do you follow a betrayal trauma–informed approach, or do you treat partners as “codependent”? - Will the therapist hold clients accountable if they deny harm or lie in family sessions? - How are betrayed partners treated — as people reacting to abuse, or as the cause of the problem? I genuinely hope The Ranch will update its approach to family involvement. Supporting someone in treatment should not mean silencing or blaming the person who was hurt. Honesty, accountability, and empathy must be part of recovery, or true healing cannot happen.

Top notch facility. This was my 3rd treatment center in as many years. I did 30 days in the PHP program. Excellent balance of treatment, individual therapy, and after care planning. The weekly alumni meetings expose you to the AA/NA environment, which is critical to maintaining long-term sobriety. The staff is caring and kind. Lots of time spent in nature, grooming horses, and social outings at the bowling alley, shops, cafes, and ice cream parlors. Cutting-edge psychotherapy (such as EMDR) is available for those also struggling with PTSD and childhood trauma. You will also feel connected to the facility leadership (the program director meets with the group once a week to address any suggestions or concerns - they proactively seek ways to better the program). Cell phones are permitted after the initial 3-day blackout period (I believe you can still make calls within the blackout using the house phone). Patients build meaningful connections with cohorts and staff. There is gym time at least 5 days a week and yoga once a week. Most of the staff are fellow addicts in long-term recovery, so they have walked similar paths of darkness and desperation as the patient…ultimately lending to an environment of trust, sharing, and growth. Overall a magical, peaceful, and fruitful experience.

I participated in the family program for a friend at The Ranch. Charlie was a great facilitator and shared excellent information on boundaries, codependency, attachment styles, and much, much more. He is very well versed in addiction recovery and the program was extremely beneficial.
