Moda Health was founded as a part of the Oregon Dental Society (ODS) in 1955 and changed its name to Moda Health in 2013. Today they offer both medical and dental insurance in four states: Oregon and Alaska along with Idaho and Texas.
Plans from Moda are available to individual and families as well as to employers. Their plans include regular medical plans and Medicare plans as well as dental coverage.
Moda Health serves over 430,000 with their medical plans and over 750,000 with their pharmacy coverage. They also have 1.3 million members in their dental plans.

Where do I begin Before I came to Elevate I thought I had reached rock bottom in life but shortly after coming here I realized they saved me from ever getting there. I came for an alcohol addiction which became a secondary objective to my stay. I learned to love myself and gained confidence in being me. I found a way to finally be happy in life, in the current moment without contingencies. To know I was never broken and that I have more control over my perspective than I ever thought was possible. My sobriety came as a byproduct of discovering my own self worth and I often lay awake at night filled with gratitude for having the opportunity to go through this program. The amount of understanding and support offered to me was outstanding and I felt as if I was amongst family. The care and love from the staff inspires the clients to care for each other in the same fashion. If at any point you need a shoulder to lean on or just someone to listen, every face you see is there for you. I can t find the words to formulate a sentence that will truly express my thanks and love for Elevate. All I can do is let others know if your suffering and need the help, this is where you will find it.

I am so grateful for the founders and all the staff at Elevate. From the very first introduction to the incredible program they have to offer throughout the entire time my loved one spent there, we were kept informed and encouraged. The setting is lovely, the commitment to helping each person succeed and get thru the hard things and the amazing food they serve as a part of the healing process. I really can't say enough good things about this program, staff and facility. Thank you from the bottom of my heart and may God bless all of you and keep you going strong for many others to reclaim the lives they were meant to live!

I m writing this as a parent. After researching several residential facilities it became apparent Peaks in Colorado Springs stood out from the others. We were specifically looking for mental health treatment. From start to finish at their facility there was always involvement in what was going on, (with my adult daughter s permission of course). Having caring and knowledgeable people to help through an incredibly hard time was needed and so appreciated. Through different stages of her care there was visitation, phone calls and weekly Zoom type meetings where families of clients could talk together with a therapist about highs, lows or concerns. The hardest thing to do was feeling like you are handing over your loved one at such a vulnerable time and not knowing what s going on. Peaks has so much compassion in knowing that. Thank you to all who went above and beyond. From inquiring to admissions to all the financial stuff to treatment to food.Thank you for your tenderness and kindness.

IT'S A YES. JUST GO.Unfortunately, an issue at home meant my stay ended early - there was a tragic miscommunication amongst staff the day before as well, which they handled with the utmost integrity. It was certainly the only snafu in my experience, and it rightfully belongs in the "sh*t happens" category. Staff had the humility and courage to own the mistake, and that willingness alone was enough to know that I had come to a place full of people who are truly the real deal when it comes to mental health.I left because I had to - not because I wanted to. In fact, I was planning on extending 30 days to complete their PHP, but life called me back home.Make no mistake - were it not for having to leave to handle something horrific back home (I'm from the other side of the country), I would still be there. I woke up this morning aching to go back to the safety, the compassion, the grace, and the mercy of Peaks. The staff are beyond wonderful. They sat with me through the darkest day of my life, and did nothing but love me. I can't describe how supported I felt. I could literally *feel* the empathy of the clinicians, nurses, and medical providers who sat with me and held space through the worst two panic attacks I've ever had.My housemates and I, and especially my roommate, wove a web of support and belonging I had never experienced. (Prior to admission, I literally could not get out of bed or do anything remotely social - I was even finding it hard to leave the house). I had the BEST time being part of a community, which gave me new confidence, and reassurance that I could get back into the game of life.I'm a therapist myself and I would 100% recommend Peaks as a committed, compassionate, caring healing space of the very highest quality. These people have their sh*t together.I have never encountered a place full of people who truly WANT to be there for you. Imagine being asked multiple times per day by staff if there is anything you need to feel supported. Being surrounded by people who know and care whether you've eaten that day, how you slept last night, and whether you need anything else to feel safe was pretty mind-boggling.I would trust Peaks to treat my nearest and dearest with zero hesitation. It will be the only mental health treatment center to which I refer my clients. Thank God for Peaks.Certainly I will forget to name some of the incredible staff, but Faith, Kinnley, Clint, Kara, Holly, Angela, Dr. Nichols, Sasha, JUDY, Cindy, Kyle, Pema, Julia, Chef, Taylor, Tara, Logan (x2), Heather, Eric, Marissa, the various therapists who led group, Bobby, Max in admissions, and Rachel - thank you for giving me a home, and at long last, an accurate mirror. Thank you also for tolerating my dirty blue hair and my massive disco fruit snuggie. I miss everyone already.Although I only got to stay for 10(ish) of the 30 days I had planned, the changes in me are real. I came into Peaks as a 47 year-old adult child, and left as a grown-up.Thank you, Peaks, for turning me back into me. Thank you for restoring my dignity. I will carry a piece of you in my heart always with immense gratitude. There is something spiritual in the particles and energy at this place that truly brought me back to life.

Hands down the best decision I made for myself to save my life. I had no idea what to do with my life as I was spiraling into a seemingly inescapable abyss until I reached out to Treehouse. I had my reservations about going to a place like Treehouse as a member of the LGBTQ+ community but the amount of support and acceptance I received even before I committed to going was incredible. I gained the knowledge and tools that I use on a daily basis that help me live life to the fullest potential. I discovered new things about myself while going through the program by living outside of my comfort zone and allowing myself to try things I never thought I would be interested in. Treehouse was able to create a space where I could explore these new things and create strong bonds with guys that impacted my recovery in the best way possible and gave me a stronger foundation to move forward with my life. The therapy was life changing, shoutout to Chuck and Ryan, they saw something in me that I could not see for myself and helped me bring that out. The coaches were incredible as well, shoutout to Dylan, he taught me so much that went above and beyond physical fitness and weight training. Vechi and Brandon were instrumental in working with me to get me the help that I needed and incredible human beings. Words cannot express how grateful and honored I feel to have gone through and graduated from this program. If you're looking for a place that'll give you the best chance at success in recovery, this is it. You are worth it.

When I first walked through the doors at Treehouse, I was lost, broken, and trapped in a cycle of addiction that I never thought I could escape, Never! I had become a prisoner to alcohol and drugs, and my life was spiraling out of control. The guys at Treehouse, there programs, and there unwavering support, helped me find my way back to the person I was meant to be!At my lowest point, I felt like there was no hope But within the walls of Treehouse, I found Compassion, Understanding, and the TOOLS I needed to fight for my sobriety. Through counseling, support groups, and the kindness of those who had walked this path before me, I learned that I was NOT alone. I discovered strength within myself that I never knew existed, and for the first time in a long time, I believed in the possibility of change. I learned to take Responsibility for my Actions, to heal from the pain I had caused myself and others, and to embrace a FUTURE filled with HOPE. Free by the Sea opened my eyes and saved my life. I would not listen to any of the negative reviews that I noticed on here. You have to come to the program with a willingness to change in order to get the wonderful treatment offered. The staff is amazing and has every clients health, integrity, and safety in their minds and care. The team is wonderful. As long as you want to change your life, you have to give in to some of the hard things that are required. 30 day residential for me was a blessing, and I am applying Friday to go back for the PHP and IOP programs.
The staff and owners expectations of you are not hard to manage if you put your mind to your treatment. The classes are eye opening, especially the Behavioral Health and Brain studies reviews. It helps you to understand your disease and how to manage it. Taking your life seriously includes steps that are mind challenging.
I completely loved my experience, was cared for, taken to a Doctor when it was needed, provided excellent meals, comfort during my stay, clothing, and other items I was lacking. They provide all hygiene products that are required. The extra classes and options for success are endless! They even go out of their way to provide you excellent transportation to and from the facility if you communicate with the admissions staff.
Sabrina provides my favorite classes! The art and especially the vages nerve classes she gave were so helpful in reopening my creative side and mind/body regulation again. I have stayed sober outside of the program, and I would suggest this resource and program to anyone who wants to change their life, live drug and alcohol free, and start over! They do not judge your past and many of the staff and counselors know first hand what you are going through. They do not take a high and mighty pose, instead they let you understand and share their stories of failures and successes to help you see you can overcome anything!
They have encouraged me in ways I didn't even know was possible! I wish I would have found FBTS many years ago, as it would have saved me from a lot of rollercoaster setbacks with my alcoholism. Put your mind and body back together in a wonderful setting. The property is an old beach resort and truly has a light hearted spiritual surrounding of nature and many blessings if you just open your heart and mind to the great experience you will have.
The meals provided are another high review from me. The kitchen staff is great, breakfast, lunch and dinner has many options, the menu is changed daily. Always salads available. Two refrigerators stocked with goods and left overs from previous meals to be eaten at any time. Full granola and cereal bar, many snacks, tea, juice machine and ice always ready. Some of the best food I have had in years, honestly high restaurant quality.
The walking track, pickleball, and men and women's lounge areas are where you can watch some TV or get some extra time with the friends you make there. You have expectations in this program and that is no different from any other situation in life. Give it a chance, change your life, get healthier while doing so.

Free by the Sea is my second treatment center. And, as a result of this program, I can truly say that I believe wholeheartedly that it will be my last. Besides the obvious, that it is set by the beach and a beautiful campus, the curriculum covered during my stay here taught me more about myself, my addiction, my trauma, and most importantly, my sobriety, than I could have ever thought possible. I do not say these things lightly. Free by the Sea taught me the importance of having a relapse prevention plan, and gave me the tools to create one. I leave here tomorrow morning, back to my husband and my life, with such solid and valuable tools to apply to my life and long-term sobriety! I can’t leave without giving a very sincere shoutout to ALL the staff who took care of me, made me feel safe, cared about, and important. Thank you, immensely, to each one, from my first step in admissions with Landon, to the front desk staff, Kandi, Kimberly, Amanda, Regina, Chanelle and Matt, to the competent and empathetic med room staff, Nyna and Courtney. The kitchen staff is amazing as well, and Kedric’s cooking is always delicious! I will forever be grateful for my experience here and the life changing and life affirming care I received. I cannot forget my counselor, Lisa, who listened, heard me, and helped me through this journey with an individualized plan. A huge thanks also to the other counselors who led the classes who gave me a wealth of knowledge; Eugene, Diane, and Brian. Free by the Sea is friggin amazing!!!! Anyone who says this place is not good for you is selfish and NEEDS REHAB!!! This place changed my life, granted it was time for me to grow up, and I will always correlate this place with an amazing time and giving me my life back. Its been extremely comforting being next to the Ocean and, comparatively speaking, other treatment facilities that I've been to do not hold a candle. Amazing food, accommodations, and staff. Especially staff; understanding, responsive and the perfect combination of thick-skinned (to deal with us adult children) and compassionate. I love you guys.

This program changed my life for the better. I would like to give a shout out to all the staff at Betty Ford, for making my dreams become possible. My counselor was Ingrid Psuty and is the absolute best. I ve been to a couple rehabs, but nothing stuck until I went to Betty Ford. Thank You so much for giving me the tools to stay sober
I just have to boast about Danny with A Better Boat Transportation. He s the angel that came all the way to my house +4 hours away to get me to Ripple Ranch. Danny is nothing short of phenomenal. He made my ride to Ripple Ranch Recovery so extremely comfortable and pleasant. He definitely eased my anxiety. Danny s communication skills are very spot on and he made me feel very comfortable. Danny is a very safe and alert driver. He was very easy to relate to and I truly appreciate him. He s a bright light in this world and a true gem. I d recommend A Better Boat Transportation to anybody! I m currently about two and a half hours from Ripple Ranch and I absolutely can not wait to get to my fresh start . I ll be sure to come back and update this review once I m done at Ripple Ranch!

There's one place on earth that I'm sincerely and honestly grateful for and that's Ripple Ranch. Coming from a man who abused drugs for over 40 yrs and I'm only 56 suddenly got tired of all the things that were slowly tearing me apart and killing me physically from my appearance to my health and seeing the scary man I was becoming made me run to God and ask for forgiveness and to truly save me cause I do want to live and wanted me family and real friend's back but most of all serve my purpose here on earth doing his will and that's when God prepared a place for me beginning at Ripple Ranch and thru that opportunity I have begun to gain access to all the important factors in life I was mistreating and taking for granted. After graduating with God leading me throughout all my days, I can genuinely say that this place cares for each and every individual that arrives for help and change and the awesome techs that work there wholeheartedly somehow pour a little extra concern into those who second guess or have hidden reservations that arise throughout there stay, and with the community (all the clients present) makes a bigger difference showing all the daily love they have for one another is just the reason that kept me involved and willing to be part of the solution and escape the real problem and for that I appreciate Ripple Ranch for giving me the chance and help me become the sober man I am today and I give all glory to God.
I was in their 2 week Relapse Prevention Program in 2003. I have been clean ever since! Facility was scenic and serene, staff was beyond amazing. Their overall approach offered total holistic focus treating the INDiVIDUAL. Diversity in spiritual accommodations including Native American. When my two week program was complete I wanted to stay longer simply because the entire experience was so profoundly healing....I WANTED more, rather than NEEDED more.

Absolutely Loved my stay at Olalla! Great meals everyday coffee too(if ya get up early enough). The staff I had during my experience were some of the best counselors & people you ll meet! Counselors will definitely call you on your BULLSHIT & keep you accountable(Can t bullshit a bullshitter, they say). So good luck trying to skate by or just faking it to make it. I got down to some good core roots I never thought of before, got a lot of Dark Things out in my time here too. Also not too bad of classes either! With a slightly more stoic approach one could say maybe, lots of good new info! Also, when you get towards the end of your stay, if you re feeling not quite ready to leave, you can ask for extension(s) too. This place is really about their patients, as groups tend to be no bigger than 6-12 people, so, plenty of questions to be asked, searched for, & answered!!! I was in the Falcon Group, our group never got bigger than 6-7 people. And it s males with males & females do there daily groups with just females, allowing for a little more comfort/support when sharing personal matters. Absolutely love Olalla & would recommend this inpatient facility to ANYONE struggling with addiction!!10/10 s

I'm going on 80 days sober because this place helped me. I loved the Trauma inpatient program. PHP was okay, I felt like it was lacking counseling and also I felt like it was non stop getting in and out of busses. You'll get your phone on lunch and like 90min on Saturday which is nice. But on Sunday when you should be calling friends and family you don't get your phone. The house on 1st street stayed nasty. I brought it up to the techs several times and nothing changed. I would give this place 5 stars but the aftercare completely sucked. I had no idea how sober houses worked or how to get into one. I had to stress out my final day because I had nowhere to go. Also I had no idea how to get a scholarship or even fill my meds... im in their IOP program currently and I'm enjoying it. Aftercare definitely needs to be worked on. I've also talked to many people after they got through with PHP and they had the same opinion. Thanksz that's my Ted Talk.

What Are Moda Health Plans?
Moda Health offers insurance plans on the state Insurance Marketplace in four states and they have Medicare plan options. They also have insurance plans that small and large employers can offer to their employees.
The individual and family plans on the Marketplace or from employers may offer a variety of benefits including addiction treatment. Most Moda plans are preferred provider organization (PPO) plans. With a PPO you don’t need a referral to see a specialist like an addiction counselor or therapist and you can get coverage for out of network providers. However if you do go out of the network you will pay a lot more out of pocket.
Moda Health also has Medicare plans. If you have Original Medicare you can get a Medicare supplement plan to help you cover out of pocket costs like coinsurance. Another option is to replace Original Medicare with a Moda Medicare Advantage plan with gives you Medicare coverage with additional benefits.
Finally Moda Health partners with Delta Dental to offer dental coverage to over 1 million members.
What Perks Are Available With Moda Health Insurance Plans?
While not all plans have the same benefits, Moda Health Insurance offers some extra perks to members that may be hard to find with other companies.
For example, there’s the CirrusMD app that allows you to contact a doctor by text, 24/7, without an appointment. You can message, share videos, and share photos in order to get more information about urgent or general health questions.
They also offer IDX Identity at no cost to help you protect your identity, including MIDAS technology that monitors health claims to detect any fraud or theft being done in your name. This gives you priceless peace of mind.
What Inpatient Rehab Does Moda Health Cover?
Most Moda Health plans cover at least some of the cost of inpatient rehab. Insurance companies focus on medically necessary care, so you must have a treatment plan from a qualified professional to show what levels of care you need for substance use treatment.
It’s also important to get the full details of your plan to ensure you get a referral or preauthorization if needed before you begin treatment.
Medical detox is one important part of inpatient care on the journey to recovery. Under a doctor’s care, you can better deal with withdrawal systems and any other negative effects of coming off the substance.
Inpatient and residential care provides an intensive, focused, structured treatment journey. It also includes access to healthcare workers and counselors who can help with any problems you face.
What Outpatient Addiction Treatment Does Moda Health Cover?
There are several types of outpatient treatment. Partial hospitalization programs (PHP) typically require at least 30 hours of commitment each week and are often known as day programs. They have a similar intensity as inpatient or residential rehab but you go home in the evenings.
Intensive outpatient programs (IOP) are more flexible, requiring about three three-hour sessions per week. You can often choose to go in the morning, afternoon, or evening depending on your work or school commitments.
General outpatient care is far less intensive than other programs. You’ll get one or two sessions per week that are about an hour each.
Does Moda Health Cover Methadone & Suboxone (MAT)?
Some Moda plans cover medically assisted treatment (MAT). Certain medications have been approved by the FDA for use in the help of recovery.
Two of these are suboxone and methadone, designed to help those struggling with an opioid addiction. At most treatment centers, you’ll have access to a doctor who can discuss if these medications are right for you, but make sure your insurance policy covers the cost and you have any referrals and preauthorizations in place before you start.
Does Moda Health Cover Other Mental Healthcare?
Most insurance plans cover mental health care because of the Affordable Care Act, but you’ll want to make sure to check your individual health plan. This can help you get treatment for diagnoses like depression, anxiety, or other concerns.
Substance abuse disorders often co exist with additional mental health problems like depression, anxiety, and PTSD. To establish long-term recovery, you will need to treat your mental health and substance use together. Fortunately, most Moda plans will cover this type of integrated treatment.
How Long Will Moda Health Cover Rehab?
Because every addiction is different and every person is unique, the length of treatment is determined by medical need. Your specific insurance plan may have its own requirement and limits as well. Your doctor will be actively involved in helping you determine what treatment to pursue, whether it’s inpatient or outpatient.
The best way to have a successful treatment journey and reach lifetime recovery is to follow through on each step of your treatment plan. Be committed to doing the work needed to create new habits and mindsets and you’ll enjoy the fruit of having a life you love.
Does Addiction Treatment Need to Be In Network?
Because most Moda plans use a preferred provider organization (PPO) network, there is likely to be coverage for out-of-network providers. However the costs are much higher.
If you choose to see someone out of network, your copayments and deductibles increase significantly, so it’s a good idea to look for a treatment center in network if possible.
How to Verify Coverage With Moda Health
Login to the helpful member dashboard or contact a customer service representative to get full details about your plan, including requirements and in-network providers.
You can also discuss your insurance plan with the treatment center. While they can give you a general idea of what treatment costs, remember that your insurance company has the final say, so they are the most reliable option. Make sure to mention your specific plan name rather than just Moda Health because every policy is different.
