Moda Health was founded as a part of the Oregon Dental Society (ODS) in 1955 and changed its name to Moda Health in 2013. Today they offer both medical and dental insurance in four states: Oregon and Alaska along with Idaho and Texas.
Plans from Moda are available to individual and families as well as to employers. Their plans include regular medical plans and Medicare plans as well as dental coverage.
Moda Health serves over 430,000 with their medical plans and over 750,000 with their pharmacy coverage. They also have 1.3 million members in their dental plans.

I think this is a wonderful rehab, the facilities are beautiful, the food is amazing (I have never even had food this good at a restaurant), and above all they have really helped my husband. Obviously anyone that first goes to rehab isn’t happy with their situation but in just two short months he was a changed person. The work they have them do on themselves got him to finally see why drugs and alcohol are no way of life. He’s finally sober and actually enjoying it! This rehab helped him so much he actually chose to stay LONGER, and I am for once excited for what the future can bring.

My experience here throughout my last few months has completely changed my whole outlook on life. Before I got here I thought my life was going towards the end. My body was failing and mental state was terrible. Every part of the program touches on every aspect of the addicts life. Now that I have finished the program and started living daily life with all of the gains I got throughout the program, failure isn’t an option. I know I can truly be happy sober now. I finally gained back the love and support from my life partner and best friend I always knew was still there. My mom also believes in me again which is so important to me. Our experience with Crestview Recovery Center exceeded every expectation we had. The level of care and compassion shown by the entire staff is truly exceptional. I came to Oregon Trail Recovery Services not as someone unfamiliar with addiction treatment, but as someone who has spent more than 15 years working in the behavioral health and substance use treatment field as a counselor, clinical supervisor, administrator, and advocate. That background gave me a unique perspective. I know what quality treatment looks like. I know the standards. I know the difference between simply providing services and truly helping people heal.
When I arrived, I was also a man who had experienced the devastating consequences of addiction firsthand. I had lost pieces of my life that mattered most, including precious time with my children. I entered treatment humbled, broken, and determined to rebuild my life one day at a time.

This experience has also reignited my passion for the behavioral health profession. Free goes beyond addiction and helps you to explore the reasons for your addiction. Most places treat the symptom but at Free they treat the symptom and the problem as long as you're open and honest with yourself.

Recently completed 30 days here. They were able to provide me with services and help in problematic areas I couldn't seem to establish outside. Staff was absolutely amazing and if ever an issue they were immediately on top of helping! Highly recommend to anyone struggling with their commitment to recovery!

Recovery unplugged changed my life in ways I never thought possible. The care for their clients went far beyond my expectations of rehabilitation centers. My counselor LuLu helped me find the understanding of myself needed to move forward from active addiction, my case manager Chloe provided me the resources needed to take the next steps, and my Alumni Coordinator Natalie has an incomparable heart for those in recovery as they take those next steps. The program also entrenches you in the Austin recovery community, intentionally becoming the connective tissue with society needed for rehabilitation from addiction. As someone who never wanted to step into rehab, I would recommend the program to anyone.

Peaks Recovery Center has truly been a life-changing experience for me.
Coming into Peaks, I knew I needed a change, but I didn’t fully understand how much this place and the people here would impact my life. The staff genuinely care about the people they work with, and they create an environment where you can actually focus on getting better, figuring yourself out, and building a better life.
What really stood out to me was the sense of community. You aren't treated like just another person going through a program. You build real connections with the other guys, share experiences, laugh together, struggle together, and encourage each other to keep moving forward. Those relationships became a huge part of my recovery.
The staff have been supportive, patient, and willing to meet me where I was at. They helped me look at things differently and gave me tools that I can take with me long after leaving Peaks.
I'm leaving with a completely different mindset than I had when I walked through the doors. I know recovery is something I have to continue working on every day, but Peaks gave me a foundation, the tools, and the confidence to keep doing it.
I'm extremely grateful for the experience, the staff, and especially the people I met along the way. If you're considering Peaks Recovery Center, I honestly can't recommend it enough.

If you are looking for help with depression, addiction, alcoholism, trauma, anxiety, body image issues… you name it. This is the place that will not only help you, they will LOVE you through it!
From the moment I called, I felt loved and cared for. Angela - You are an Angel! Angela kept in contact with me several times a day until I was on a plane to CO. As soon as I got there, Katie picked me up from the airport, she was a doll! She made me feel so comfortable going in. I met with a few staff members, and stayed in “detox” for 2 days.
When I first got to the Women’s house, I met Miss. Pat - She’s the most incredible woman I’ve ever met. I’ll never forget her. EVER.
I went in for Trauma and Depression. Within 7-10 days, I could tell my life was changing. I had never felt this good in my life.
Chris, Bobbie, and his team hired a staff that is like nothing I would have ever imagined. From the Director (Sara), Doctors, Therapists/Counselors (Kate, Madi, Shayna, Sam), PA’s (Alyson), Gym Instructors (Nicole and Dan), my caseworker, Tyler, the chef, Taylor, and the staff (Miss. Pat, Cassie, Leah, Kayla, Mariano, Julie, and Cheyanne), - They are all AMAZING! Everyone has a heart of gold!
Each week in group they discuss a topic. The topics discussed are LIFE CHANGING! There are things in life that you think you know, you don’t. I learned so much in group and I’ll hold onto that info for life.
I also met with my Therapist/Counselor Kate, 2-3x a week. Kate is part of the reason I am who I am today. ❤️

It all started with a call to Hew he flew me out the next day he would have flown me that day I was just so nervous I got here Dec 31, 2024 it was close to midnight No one was banging pots & pans for the New Year they were there to take care if you. I am literally crying everyone the Techs, nurses, nurse practioner, cook, therapist case manager These are the group of people that were exceception!! you know how some people you just don’t like them annoying especially going threw this I had a nurse Rose a beautiful Tech they sat up with me two nights my pressure was down I couldnt take anything a nuse practioner Mia no matter where you were or what time it was She was there & fighting for you she made sure my insurance was giving me the shot I needed Kyle my therspist all the therapist whobwe had for group, we were changing our lives they helped us threw the day taught us so much, even planting days out movies, bowling, Ben our guy to take us to meetings, dont play Taylor Swift lol but he did take us to 7-11 for a snacks I could go on & on and on, nurse supervisor I dont want to destroy her nameI’m a bad speller, lol Mark tough on me and I needed it, , the Techs OMG you know who you are !!! much love the cooks let say this I gained a lot of weight , movie theator, gym, Not to forget the group of people I took this journey with I am still in contact with them we supported one another we cried laugh together we were all there to break these chains that were holding us down we broke the chains together I am forever grateful to all of these people that were a part of my journey we still have or group chat these are the people I want in my life changed the people & fridnds, check !!
You know I don’t even remember how I found this place my dr had me on a lot of pills for decades ir just got worse I needed a change God knows what would have happened it woud’nt be good all I wanted was to get out of Jersey far away that I couldnt run from I didnt see the pictures of this place, Calorfornia here I come When I got off the plane there was a tech holding my name & so we begin
These pictures they posted believe me there is so much more the grounds stunning relaxing the pool sitting there seeing the mountains, you can see a horse being trained, the turtle on the side of the house, the goats one guy was from Wyoming his face just glew he never seen or fed goats after the detox your transferred to another house O M G I felt like a Kardasion no kidding my room double doors to a patio I just sat in peace seeing the beauty of these mointakns I’m a city gal, the hummibirds Extraordinary at night 3 bunnies came every night I can hear Ben yelling not to feed them at the time I had 3 grandchilren I missed them so much I named them after them & they honestly they gave me much comfort… ok I could right a novel about this place if you need to fix your life break those chsins GO HERE your going to get that help break those chains in a serene environment with these people that are going to change your life isnt that what were seeking make thst call you will not regret it I’m still here in the mental health program working as a house manager I did it you can do it ..

Bayview is a wonderful facility with a professional and caring staff, who are support in your goals and helping you to accomplish them. The process was easy and professional from checking in to graduating the program. The facilitators of sessions are engaging and try to further deeper thoughts for conversations and the counselors are the best I have had the pleasure of working with. I would recommend this program to anyone seeking a new journey.

Does Moda Health Cover Addiction Rehab?
The Affordable Care Act requires that most insurance policies cover mental and addiction treatment, but every policy is different. You can learn your exact amounts based on your policy by discussing it with the treatment center.
Insurance often focuses on medically necessary treatment like detox and medication. If you don’t have a specific treatment plan in place, reach out to a qualified healthcare professional to create a long-term plan to submit to your insurance.
What Are Moda Health Plans?
Moda Health offers insurance plans on the state Insurance Marketplace in four states and they have Medicare plan options. They also have insurance plans that small and large employers can offer to their employees.
The individual and family plans on the Marketplace or from employers may offer a variety of benefits including addiction treatment. Most Moda plans are preferred provider organization (PPO) plans. With a PPO you don’t need a referral to see a specialist like an addiction counselor or therapist and you can get coverage for out of network providers. However if you do go out of the network you will pay a lot more out of pocket.
Moda Health also has Medicare plans. If you have Original Medicare you can get a Medicare supplement plan to help you cover out of pocket costs like coinsurance. Another option is to replace Original Medicare with a Moda Medicare Advantage plan with gives you Medicare coverage with additional benefits.
Finally Moda Health partners with Delta Dental to offer dental coverage to over 1 million members.
What Perks Are Available With Moda Health Insurance Plans?
While not all plans have the same benefits, Moda Health Insurance offers some extra perks to members that may be hard to find with other companies.
For example, there’s the CirrusMD app that allows you to contact a doctor by text, 24/7, without an appointment. You can message, share videos, and share photos in order to get more information about urgent or general health questions.
They also offer IDX Identity at no cost to help you protect your identity, including MIDAS technology that monitors health claims to detect any fraud or theft being done in your name. This gives you priceless peace of mind.
What Inpatient Rehab Does Moda Health Cover?
Most Moda Health plans cover at least some of the cost of inpatient rehab. Insurance companies focus on medically necessary care, so you must have a treatment plan from a qualified professional to show what levels of care you need for substance use treatment.
It’s also important to get the full details of your plan to ensure you get a referral or preauthorization if needed before you begin treatment.
Medical detox is one important part of inpatient care on the journey to recovery. Under a doctor’s care, you can better deal with withdrawal systems and any other negative effects of coming off the substance.
Inpatient and residential care provides an intensive, focused, structured treatment journey. It also includes access to healthcare workers and counselors who can help with any problems you face.
What Outpatient Addiction Treatment Does Moda Health Cover?
There are several types of outpatient treatment. Partial hospitalization programs (PHP) typically require at least 30 hours of commitment each week and are often known as day programs. They have a similar intensity as inpatient or residential rehab but you go home in the evenings.
Intensive outpatient programs (IOP) are more flexible, requiring about three three-hour sessions per week. You can often choose to go in the morning, afternoon, or evening depending on your work or school commitments.
General outpatient care is far less intensive than other programs. You’ll get one or two sessions per week that are about an hour each.
Does Moda Health Cover Methadone & Suboxone (MAT)?
Some Moda plans cover medically assisted treatment (MAT). Certain medications have been approved by the FDA for use in the help of recovery.
Two of these are suboxone and methadone, designed to help those struggling with an opioid addiction. At most treatment centers, you’ll have access to a doctor who can discuss if these medications are right for you, but make sure your insurance policy covers the cost and you have any referrals and preauthorizations in place before you start.
Does Moda Health Cover Other Mental Healthcare?
Most insurance plans cover mental health care because of the Affordable Care Act, but you’ll want to make sure to check your individual health plan. This can help you get treatment for diagnoses like depression, anxiety, or other concerns.
Substance abuse disorders often co exist with additional mental health problems like depression, anxiety, and PTSD. To establish long-term recovery, you will need to treat your mental health and substance use together. Fortunately, most Moda plans will cover this type of integrated treatment.
How Long Will Moda Health Cover Rehab?
Because every addiction is different and every person is unique, the length of treatment is determined by medical need. Your specific insurance plan may have its own requirement and limits as well. Your doctor will be actively involved in helping you determine what treatment to pursue, whether it’s inpatient or outpatient.

Does Addiction Treatment Need to Be In Network?
Because most Moda plans use a preferred provider organization (PPO) network, there is likely to be coverage for out-of-network providers. However the costs are much higher.
If you choose to see someone out of network, your copayments and deductibles increase significantly, so it’s a good idea to look for a treatment center in network if possible.

Login to the helpful member dashboard or contact a customer service representative to get full details about your plan, including requirements and in-network providers.
You can also discuss your insurance plan with the treatment center. While they can give you a general idea of what treatment costs, remember that your insurance company has the final say, so they are the most reliable option.
