Aftercare planning may include continued individual or group therapy or continued medication assisted treatment. Some clients may transition to a sober living program. The sober living program through Evoke supports individuals who require continued care and relapse prevention. It provides an environment where the benefits include a greater ability to remain sober, higher odds of finding employment and staying employed and improved recovery from other mental health issues that they may be experiencing.

this was the worst possible place for me to go when i was struggling with my mental health. it is mostly a rehab program for addiction, though it was marketed to me as a blend of mental health and addiction treatment. the directors were unprofessional and refused to let me leave when i realized the program was not helping me, even though i was there voluntarily and SIGNED PAPERS saying i was there voluntarily and could leave whenever i wanted. i also heard that after i left, someone reported sexual misconduct to the directors and they dismissed the person and told them they didn't believe them. disgusting. i also told them i needed housing other than going back to my abuser's house, and they told me that was not possible, other than a sober living house. why would i go to a sober living house if i was not struggling with substance abuse? they don't get to know you or your problems. you only get individualized therapy once a week. oh, and everyone is on drugs. like cough syrup and shit. i left in a much worse state than i came in. terrible place!

Amazing program, staff are caring and great at their jobs. Michael p is an amazing clinician, very supportive with mental health and addiction. There night I.O.P was very helpful in my recovery. The groups are amazing. the program is great and the staff actually care about you.

These programs are an integral part of most relapse prevention plans and provide continued support and accountability that support sobriety.

24 hour clinical care provides close medical supervision for individuals in the early stages of recovery. Medical professionals monitor individuals around the clock for complications and provide immediate intervention when needed.

Intensive outpatient programs at Evoke Wellness provide the highest level of care with a flexible schedule. Individuals work with healthcare specialists to create treatment plans to meet each person’s unique recovery needs. Care includes individual and group therapy, medically assisted treatment when indicated and long term recovery planning. 

Partial hospitalization programs are the highest level of outpatient care. This is for individuals with moderate to severe mental health and co-occurring disorders that need the flexibility of going home each night. These comprehensive plans require participation several days per week for a large portion of the day and include individual and group therapy.
