Administration is so rude and unwelcoming . Specifically Demetria the scheduler . She’s a bully and she picks on agency staff . Very rude and unwelcoming . Nobody speaks to you and nobody says a word to you .

In individual therapy, a patient meets one-on-one with a trained psychologist or counselor. Therapy is a pivotal part of effective substance abuse treatment, as it often covers root causes of addiction, including challenges faced by the patient in their social, family, and work/school life.

During multiple visits to see my uncle, there has been a strong smell of urine and feces throughout the facility, which is very concerning. On one occasion, we also saw a mouse run across the floor near our feet, which raised serious questions about cleanliness and pest control. In addition, my uncle is only being showered about twice a week, which does not seem adequate for proper hygiene and care. Overall, these conditions are unacceptable and need immediate attention. Residents deserve a clean, safe, and properly maintained environment, and I hope management addresses these issues urgently.

I rarely leave negative reviews, but my experience with this facility raised significant concerns regarding resident safety, communication, and overall quality of care. My resident was frequently left in soiled briefs for extended periods, reportedly due to short staffing. Each shift (day, evening, and night) appeared to blame one another, but the lack of consistent care remained unresolved. The facility also had no hot water for nearly two weeks, which resulted in residents being washed with cold water. In some cases, staff reportedly microwaved water to attempt basic hygiene care. This raises serious concerns about infection control, dignity, and resident comfort. Communication and follow-through were also extremely poor. Phone calls were not returned, and requests from families went unanswered. The assigned social worker, Heather, did not provide follow-up, did not communicate with families (always said she would look into the resources, but then she always reported back with the same repetitive conversation with no answers. Heather also did not ensure discharge planning needs were completed. Prior to a scheduled Saturday discharge, I specifically requested during the clinical meeting confirmation that home health services, PT, OT, durable medical equipment, and necessary resources would be arranged. Despite leaving two messages during the week, nothing was completed (The resident is a fall risk usually need a cane or walker, sent home with nothing, only prescriptions). There was no follow-up, and no services were arranged, leaving us without support upon discharge. As a nurse with 26 years of experience, I have never encountered such a lack of coordination, accountability, and responsiveness. Residents deserve safe, dignified care, and families deserve clear communication. Such a horrible experience and just because my resident was discharged, I am not done with this place. There is so much more I could say, I could honestly write a book about the issues observed from the very beginning of admission that I consistently advocated about. What concerns me most are the residents who do not have strong advocates. Even with consistent and persistent advocacy, services were still not provided as needed, which is deeply troubling. I genuinely fear for those without family or support speaking on their behalf. While I want to acknowledge that some staff members did try, particularly Physical Therapy and Ms. Pat the GNA, who provided compassionate care, the overall system and nursing has a lot of improvement to do.
