Integrated Outpatient Services Include Primary Care

Case Management helps you manage your recovery plan and also helps you access other needed resources in the community. Their case manager meets with you to help you evaluate your service need, to help you develop your individualized treatment and recovery plan and acts as a coach to encourage you along your plan. Your case manager meets with you at scheduled times, helps monitor your progress and quality of services received and assists you in making connections with other supportive community resources important for your recovery process.
