Please do not send your loved one here. This place is NOT well run. My mother was there 2 1/2 months. She fell 3 times in 3 days. Her face was bruised and her eye was swollen shut yet wasnt sent to the hospital. The last fall she shattered her hip. I could not in good conscience send her back there. Very poorly run, and overall rundown.

My grandfather attended rehab at this facility. The rehab staff was amazing and the Director Savanna is wonderful! They always have great communication. He is now there long term. At first I was apprehensive about him going there and reading some reviews. But this building has been amazing! The male nurse on the second floor is very knowledgeable and patient with my grandfather.

Treat my grandmothers with dignity and respect. Nurses are kind and update me on anything going on. CNAs take great care. The build is clean and free from odors.

Do NOT send your loved ones to this facility! I was verbally assaulted by one of the employees in the parking lot. He told me he was the Maintenance Director. After I tried to calmly understand what he was trying to tell me, he proceeded to raise his voice and get in my personal space to berate me. This employee should be fired for how he handled this situation and ashamed of himself for speaking this way to a young woman. I can only imagine how patients are being mistreated here after what happened to me. I am appalled and I feel violated.

Over the years I have dealt with several injuries that have required multiple surgeries, physical therapy, and specialized medical equipment in order to heal. Last year both of my shattered hips were replaced, which went fine until a staph infection necessitated the removal of my right replacement hip. After my hospital stay I was admitted into Maplewood Rehab with a laundry list of specialized medical needs that needed to be tended to daily in order for me to recover. I was admitted unable to walk due to the missing hip. I had a wound vac attached to my right side that required specialized training and constant attention. I have a PICC line to receive IV antibiotics 3x/day. All of these needs were addressed with competence, kindness, and adherence to protocol. The staff here is professional, kind, and invested in my healing despite the extra workload that my care requires. Nurse Ladonna and nurse Jessica are angels for examples. PT Dawn and OT Jessica have helped me to get stronger, and relearn the life skills needed to go home to my family. CNA Rhonda and her colleagues take care of me like I'm family. The culinary staff takes pride in their work. The spicy chicken and the pizza are two of my favorites. Angels, the whole lot of them. If you or a loved one need skilled nursing rehab after a grievous injury, the staff at this facility will take excellent care of you. Additionally, the staff here have a very good rapport with my wife, as well as my team of doctors in Boston. And this broken down old EMT is grateful. Chris Janvrin UPDATE: Yesterday I was able to finally go home to my family. This has been a three month ordeal, but at least I am strong enough now to heal at home until my next surgery. I am home today because the staff at Maplewood Rehab made it their mission to help me get stronger. As I said before, angels.

The staff is really nice here and the patients too. I will miss the friends I made. The therapy was very helpful. All therapist are very nice. Jessica the LPN and Rhonda, Anne and Luanne CNAs are all amazing on the first floor. Activity staff are all great as well. I recommend Maplewood.

If I could give this place zero stars, I would. My mother came here from the hospital after a fall, and it was terrible from the start. She was admitted Monday night. I asked about pain management for a compression fracture in her spine, and was told they'd call the doctor about it. Tuesday I went to visit and she hadn't been evaluated and still hadn't received pain management. She was in a lot of pain and needed meds. I asked the nurses again, and once again was told they'd call the doctor. That evening I noticed my mother was sitting in an old, wet diaper. I spoke to a nurse who gave me an attitude saying she was changed that morning... This was 5pm, she should have been changed hours before. I again, asked about the doctor and pain management and was told they'd give her Tylenol and call the doctor. As I was sitting with my mother I was watching how the interactions were between other patients and staff, and I didn't like what I saw. Dirty residents, residents being spoken to with irritation, and one that was being led pretty aggressivly by the arm somewhere. You could tell it was too fast for her. My mother was supposed to be fed her meals and they just left trays there. She has alzheimers/dementia and wasn't able to feed herself. She was also complaining about pain in her stomach, which I wanted addressed by the doctor. I decided I wanted my mother out of there, and as soon as possible. Wednesday morning I reached out to another facility and set up a tour. It was clean, didn't smell, the residents weren't all over the place, the nurses and aides were all friendly and management gave me so much information that I never received at Maplewood. I felt happy leaving there. It had a much more homey feel to it. Maplewood was disgusting. It's dirty with smears of who knows what on the walls, spills on the floor, and the floors were sticky. The furniture is dirty, old and falling apart. There was also an overwhelming smell of urine. The place feels messy, chaotic and cold. The staff is rude and have horrible communication skills. They always looked bothered when I'd ask about her needing meds, or needing to be changed or for anything really. I went in and told them I wanted to get her into another facility and have it ready and set for them to work on the transfer. I was told not to go to this other facility because they were closing (not true), they weren't good, and they have some ex employees of theirs, and I might want to make sure I want her to go there. I tolf them yes, I want her to go there. The nurse was arguing with me that my mother was changed, and I don't believe she was. Not with how old that diaper looked and how full it was. Everyone that was cold and unfriendly before became friendly and accommodating on the nursing floor. Everyone in the lobby and management that were initially friendly while trying to get us in there, turned cold and unfriendly after I wanted my mother moved. So much for being professional. They even tried to get me to take my mother to the new place on my own, knowing she'd be leaving against medical advice and it would mess up her Medicare. There were maybe two nurses that were friendly but overall, most of the staff was unwelcoming and seemed annoyed with their job. I heard from the social worker this morning telling me she was just sitting down to get to my mother's admission forms and saw a note that I was transferring her and was I sure they took her insurance, and did they even have space for her? It's Thursday... My mother was sent to their facility Monday night at 5pm and you're just now getting to her file? There was no care plan made, nobody called to speak to me about her medical chart or how they planned to take care of her. For someone that was admitted with a compression fracture and in pain, they failed at adequately caring for her. If you can avoid Maplewood, please do!

This place took great care of me. The help is wonderful here and I recommend it for rehab. Jessica is a fenominal nurse. All therapy staff are also fenominal.

I'm so appalled with this place and all the violations broken within this facility, just the smell of the urine even before you enter the doors you can smell it, it's rancid- fact of the matter is,is that nobody should be breathing in airborne urine germs, I also personally had a security breech happen, there was one receptionist that was too lazy or incapable on actually getting out of her chair so I ended up letting myself up to my loved one's room, they're way(s) of care for their patients is deplorable I'm sorry but it negligence for aides/nurses to leave their patients in their soiled clothes from overnight into late morning- close to noon, my loved one was told to tell me I wasn't allowed to call there right before his second sleep in there, my first visit our friend went up with me, my loved one had a difficult time swallowing he pulled six whole pills out of his mouth- left unsupervised, omg this place caused me a TREMENDOUS trauma, it's left me scarred. I've put out more than several calls, left messages, and it took THREE MONTHS to get through- it got me nowhere, my loved one came back as a different person in words I can't being to even explain, if anything the impact it left in my loved one, took a toll on him, there was only two staff employees that we both truly liked. They need to do better training and in a timely fashion, their violations need to be attentive to they're own caused Repercussions with safety violations, sanitary violations, hygiene violations, maintaining with the cleaning staff need to do it more often, retrain or hire people worth letting them earn an honest paycheck, I'm not too concerned with writing an open but honest post with an awful traumatic time for myself and my loved one, this place left me permanently scarred, and it paralyzed us both of us mentally. This place is no good, and again if I could give it zero stars I would. I'm sorry for reading other low reviews but I absolutely believe all of them, it makes me incredibly sad to see patients and loved ones go through

Horrible Horrible place it's dirty , furniture falling apart ,my mother's clothes are missing , smells disgusting i was in shock when I went to visit her today and tomorrow morning I will be calling and reporting them to D.P.H and EOEA I have never been so angry in my life
