Freedom Health is a managed care organization (HMO) that offers Medicare Advantage insurance plans in Florida. Plans are available to people who are eligible for Original Medicare due to their age or disability.
Freedom Health also offers chronic special needs plans (C-SNPs) and dual special needs plans (D-SNPs) that are available to people who have both Medicare and Medicaid.
Freedom Health’s headquarters are in Tampa, Florida. The company was founded in 2005 and currently has more than 62,000 members throughout 24 Florida counties. As an HMO, Freedom Health utilizes its own network of doctors and specialists to provide services to members.
Freedom Health’s provider network consists of primary care doctors, specialist providers, and pharmacies. Freedom Health has a Medicare rating of 4.5 stars (out of 5) for its Medicare Advantage plans.
Does Freedom Health Cover Drug Rehab & Addiction Treatment?
Freedom Health Medicare Advantage plans do cover mental health and substance abuse treatment in addition to medical treatments. Substance abuse treatment options will differ depending on the policy chosen. Copays or coinsurance may apply. Some policies may also have limitations on certain services.
Freedom Health plans may require you to use doctors and centers in the plan’s network to get coverage, so be sure to check out your plan’s specific details before you commit to treatment. For those plans that have out of network coverage, you’ll pay quite a bit more in out of pocket costs if you use a non network provider, so keep that in mind.

As an attorney, I referred my client to Fifth Street Counseling and the results were outstanding. They worked towards providing a tailored program for my client and my client consistently praised the staff’s professionalism, empathy, which provided a safe and transformative environment for growth. It is rare to encounter a facility so committed to excellence in care with the kindness they show. The communication with my office was seamless and prompt with any outstanding or follow up questions I had. Both Garry and Adam were a pleasure to work with. I wholeheartedly recommend Fifth Street to anyone seeking professional and compassionate counseling and treatment services.

I’m not a patient, I bring family members here, so this is a caregiver’s perspective. Both patients I bring express that they’ve connected well with their therapist and Dr. They are benefiting from services, which are offered on a sliding scale for those without insurance. May be $0 for people with no income and no insurance. I’m grateful these services are available in our community!! If I ever need to, I will go here.
And Heather, (front office) you are always so kind and so helpful. We appreciate you so much!

Our daughter is there for 3 weeks now and is showing more signs of improvement than we saw in years. Dr. Moran and the team really know what they are doing and have tools and desire to make a difference. Plus now we are more equipped then ever to keep on track for recovery once she comes back home, thanks to all education we received from FCRF. I finally have hope! I know if you are reading these reviews, you are desperate, same as I was a month ago. You don’t need to be, don’t wait! I recommend this place without reservation!
Update: it has been almost 2 months after my daughter left FCFR and it’s the healthiest 2 months we’ve seen in a long while. She hasn’t used any substances, found a part time job which she loves, graduated her failing semester with As and Bs and about to get her drivers license! It’s not all rainbows and butterflies, we still need to work hard daily on keeping things on track, but now the hope is getting more real every day!
10 months update: FCFR has truly been a game changer for us! Now my daughter is just a difficult (normal) teenager, but the disaster and desperation is so far behind, it’s almost hard to recall how it has been. Now she is committed to getting better and going on with her life, and that’s the most important part. Of course there will be ups and downs, but the terror of being constantly afraid for her and of her is far behind and I never ever want to go back. If we ever go back (it gives me anxiety to even write those words), we know who to call.
One last thing. There are a lot of comments about lack of tender, love and care at the facility. TLC FCFR does not provide indeed. But they provide something better: treatment THAT WORKS. I’m sure every story is different, but I’m also sure a lot of people have experienced the same thing I did: TLC does not treat psychiatric illness.

The police ministry at this center has been a blessing for my life. The guest speakers they bring in every month have nurtured me spiritually and everyone at this place is so welcoming and kind. The head minister Jorge is a great man of God and he works very hard to spread the Gospel of Jesus Christ that is so wonderful.

With Deepest Gratitude to Southern Winds Hospital
There are moments in life when words simply aren’t enough—and this is one of them. Our family will forever hold a special place in our hearts for Southern Winds Hospital and every single person who walked alongside us during one of the most painful and uncertain times in our lives.
After our beloved father suffered a brain stroke, worsened by arthritis in his neck, he developed dementia. We were devastated, unsure of what the future would hold, and feeling lost. But from the moment he arrived at Southern Winds, we were embraced by a team whose compassion, dedication, and humanity gave us something we thought we had lost—hope.
From the front desk staff who greeted us with warmth and friendly smiles, to the security team who made us laugh with gentle jokes—knowing our hearts were hurting—every interaction brought comfort. The cleaning crew, nurse technicians, and every staff member who passed by us made sure we were okay. They didn’t just care for our father—they cared for us, the family, with genuine kindness.
Every day, our father was bathed, shaved, and dressed in clean, regular clothes. He smelled fresh, looked peaceful, and was clearly being cared for with love and dignity. These small but powerful gestures reminded us that he was being treated not just as a patient, but as someone deeply valued.
We especially want to thank Social Worker Ernesto, whose guidance and support were a lifeline. Ernesto took the time to explain everything clearly, helping us understand the steps needed to bring our father back home. He was patient, compassionate, and truly committed to our father’s well-being. Alongside Ernesto, the physician assistant, nurses, caregivers and the doctors treated our father as if he were their own father or grandfather—with tenderness, respect, and unwavering care.
The medical team kept us informed every single day, calling us with updates and making sure we felt involved and reassured. Their dedication went far beyond duty—it was personal, and it meant the world to us.
If we could list every single name of the staff members who touched our hearts, we would end up writing a book. That’s how many people made a difference. From the smallest gestures to the most critical decisions, every act of care mattered.
To the board of Southern Winds Hospital: please recognize the incredible team you have. The doctors, nurses, social workers, technicians, and support staff are not only skilled professionals—they are healers, comforters, and heroes. They gave our family peace, hope, and the gift of more time with someone we love deeply.
Today, our father is home. He is responding to treatment, showing signs of joy, and doing better than we ever imagined. Even with the challenges of dementia, he is thriving—and we truly believe that the love and care he received at Southern Winds played a vital role in his recovery.
To everyone at Southern Winds Hospital: thank you. Thank you for your kindness, your professionalism, your laughter, and your heart. You gave us more than medical care—you gave us healing, hope, and love. We will never forget what you’ve done for our family.
With all our love and gratitude,
From a Family Forever Thankful

I do not have many bad things to say about this place. Yes, it is an old building, but the staff and process were not a problem for me.
I was there for 15 days and the last 7 were voluntary.
Yes, the towels are small and the toiletries are basic, but the staff while I was there left the towels out so you could grab as many as you needed (def need at least 2). The snacks were plentiful and of a good variety. The staff were very friendly and caring and felt like they really cared about you getting better.
They also allowed for outside time on the patio or out in the courtyard. We had outside time almost daily, if not daily. The weather was just beautiful each day and a much needed warm up bc they do keep it cold (bring a jacket/sweater if u can). The staff also let you freely go to your room if you were cold, which I've been at a different facility where they did not allow you in your rooms during the day.
They offered music therapy while outside, groups during the week which included a guided medication, a session with the Chaplin and their in-house massage therapist who led us in some light stretching one day on the patio. We also had gym time almost daily where you can play basketball, volleyball, access to yoga mats, and more music.
The most valuable thing for me was them letting us have access to our rooms any time of the day bc I was very cold much of the time and did not have a sweater and the sitting room does stay cold. Also, especially if someone is extra groggy bc of meds, they dont force you to sit thru it in the sitting area like other facilities. This was so valuable to me who is someone who is sensitive to meds and Not a morning person. They also do rounds to every room about every 10 min to keep close watch over the patients, which undoubtedly saves lives.
And they have plenty of everything so do not feel shy about asking for extra. Just "ask and ye shall receive". And I did not get the religious pressure like another review reported.
Going to the cafeteria was the highlight of the day. I thought the food was good and for being there 15 days, they never duplicated a lunch or dinner meal. The variety was amazing. But they do keep it healthy, and rightfully so. So low sodium, low fat, low calorie. No weight gained, no weight lost. Coffee is also available every morning and in the cafeteria at breakfast, which was also a plus.
It is an older facility and the beds are not great, but you can ask for an extra pillow or blanket and they have cleaning staff come thru daily to keep things clean and there were no bugs. I saw them pressure washing the outside areas twice while I was there.
The last thing I will say that was a con was the nurse that dresses in all white. She does not seem to be very useful for anything other than trying to hinder your progress so stear clear of her if you can.
Otherwise, I would recommend this location just as well as any other, if not more so. I gave it 4 stars only bc of the age of the facility (which honestly cant be helped) and the less than stellar toiletries/showering experience (the shower curtain is affixed with velcro so it does come loose easily and water will flood the floor outside, the small towels, and sometimes the hot water will run low and may have only a warm shower).
I am very thankful for the staff here. Thank you to those who go above and beyond, I hope you know who you are.

Frequently Asked Questions
What Types of Inpatient Rehab is Covered By Freedom Health?
Drug rehab treatment options that may be available through Freedom Health’s Medicare Advantage insurance policies include a range of inpatient services.
Detoxification to remove drugs from the body, as part of alcohol or opioid addiction treatment, may be covered through some Freedom Health insurance plans. During detox, typically one of three FDA-approved medication-assisted treatment (MAT) medications–methadone, naltrexone, or buprenorphine–are used. Detox treatment is often administered as part of inpatient hospitalization.
Inpatient drug rehab in a clinic or a hospital may be covered by Freedom Health, just as it is available through Original Medicare Part A. Inpatient care must be verified as medically necessary by a physician.
Inpatient mental health treatment is also part of Freedom Health plans, but it also must be verified as medically necessary and you’ll need to choose a clinic that is approved by Medicare and accepts your specific insurance plan.
What Types of Outpatient Rehab are Covered By Freedom Health?
When you’ve completed inpatient treatment, or if your doctor recommends that you start with outpatient care, there are a variety of programs that may be available to you.
Partial hospitalization programs (PHPs), which are also called day programs, are delivered as outpatient treatment in community mental health hospitals or community mental health centers. In PHP programs patients attend sessions during the daytime and return to their homes at night.
PHPs following Medicare guidelines must be directly supervised by a physician. Treatments must be outlined in an individualized care plan ordered by the doctor. Most PHP sessions last 4-8 hours a day and take place over three to five days a week.
A total of at least 20 hours of treatment per week for 3-4 weeks are typical. However, treatment can last up to two months or more, if the treatment plan specifies it. PHP treatment includes individual, group, and family counseling, along with addiction and relapse prevention education; it may also include mental health medication when it is needed.
Intensive outpatient programs (IOPs) often include 9-20 hours of treatment per week, with these hours spread across 3-5 days. A recommendation of 90 days of treatment is typical for IOPs.
IOP programs include individual, group, and family counseling sessions, plus addiction and relapse prevention education. 12-step programming may be offered. Some intensive outpatient programs are structured as opioid treatment programs, which have been covered by Medicare since 2020.
General outpatient care meets once or twice a week and can help you maintain and reinforce everything you’ve learned in previous levels of addiction and mental health treatment.
Medicare Part B covers telehealth services by phone or internet when these services are provided by a physician or a mental health counselor who is contracted with Medicare as a provider. For services that treat addiction, mental health, or both, you don’t have to be in a specific area; instead, you can receive telehealth treatment from home.
Because Medicare Advantage plans must cover at least as much as Original Medicare, your Freedom Health plan will also include these telehealth options.
Are There Additional Benefits Available With Freedom Health?
Many Medicare Advantage plans offer additional benefits that Original Medicare doesn’t have. Freedom Health is one of the company that offers a variety of extra benefits and coverage.
For example, they have premium reimbursement for some Medicare Part B fees, which is returned to you in your Social Security check. Freedom Health plans also provide free wellness checks and screenings, 24/7 access to a nurse advice line, and a Free Silver Sneakers® fitness program membership.
You may also get a monthly pharmacy allowance of up to $125 for over the counter drugs and transportation to provider visits. Members of the special needs program (SNP) plans have access to caseworkers who can help you manage finances, keep your home safe, access the medication you need, and connect you to other resources.
What Are Freedom Health Plan and Care Levels?
Freedom Health offers 3 types of Medicare Advantage plans. They are Health Maintenance Organization (HMO), Special Needs Plan (SNP), and Point of Service (POS) plans.
Their HMO plan offers participants the ability to choose their own primary care provider from a list of participating health professionals. Should a patient require the services of a specialist the PCP is able to send a referral to that provider for coverage. Some of these plans have no monthly premium costs and have low copayment requirements.
The SNP plan is for individuals who qualify for Medicaid or Medicare due to a chronic illness regardless of age. This plan helps individuals to offset the out of pocket cost of their medical treatment and prescription costs. Some of these plans have no monthly premium costs and have no copayment requirements.
Freedom Health’s POS plan offers the participants the option of seeing both in network and out of network providers and hospitals. The amount of coverage is greater when individuals see in network providers. For out of network care there is a higher out of pocket cost. Referrals to specialists may be required from the patient’s primary care provider.
