Does Alcohol Cause Kidney Stones?

Dehydration is the biggest factor. Alcohol is a diuretic, and it suppresses the hormone that tells your kidneys to retain water, so you lose more fluid than you take in. Concentrated urine is the primary environment where crystals form and clump into stones.2,3

Uric acid load is the second pathway. Alcohol, especially beer and spirits, raises uric acid levels in your blood. Uric acid stones are directly tied to this, and high uric acid also worsens conditions that lead to calcium oxalate stones, the most common type.2,3

Disrupted mineral balance is the third factor. Heavy drinking interferes with how your body handles calcium, phosphate, and magnesium, all of which play roles in whether crystals remain dissolved or precipitate.2,3

How Alcohol Increases Kidney Stone Risk

Alcohol affects your kidneys through three distinct mechanisms. Understanding them explains why drinking, especially heavily or regularly, raises your risk of developing stones.

Dehydration and Concentrated Urine

This is the most direct pathway. Alcohol suppresses antidiuretic hormone (vasopressin), the hormone that signals your kidneys to hold onto water. Without it, your kidneys produce more urine than normal, even when you are not drinking enough to compensate. The result is dehydration and, critically, highly concentrated urine. That concentration matters because kidney stones form when dissolved minerals such as calcium, oxalate, and uric acid reach levels too high to stay in solution.4,5

Purines and Uric Acid Stones

Beer and many grain-based spirits are high in purines, which break down into uric acid. When uric acid levels in your blood rise, your kidneys filter more of it into your urine. Combine that with concentrated urine from dehydration, and you have conditions in which uric acid stones form.6

Beer can create a significant purine load combined with the volume and diuretic effect of alcohol, hitting uric acid levels and urine concentration simultaneously.2

Spirits

Grain spirits, like whiskey and bourbon, carry elevated purine content, though without the same volume factor as beer.2

Wine

Wine sits in murkier territory. Some data suggests wine drinkers have a lower risk of kidney stones.2 One explanation may be that wine is often consumed with food and water and in smaller volumes, rather than anything uniquely protective in the wine itself. The most important thing you can do is also the simplest: drink less. People who cut back or stop drinking typically see measurable improvements in hydration markers, blood pressure, and kidney function within a short period.

Other steps that help:

  • Stay hydrated while you drink. Aim for one glass of water per drink, and make a point of drinking extra water the day after. This directly counteracts the diuretic effect and keeps your urine dilute enough to prevent crystal formation.10
  • Rethink what you drink. If you have had uric acid stones, beer and grain spirits deserve particular caution due to their purine content. Wine, consumed in moderation with food and water, carries a comparatively lower risk profile. Lower risk does not mean no risk.
  • Eat when you drink. Small, balanced meals support slower alcohol absorption, which moderates the dehydrating effect. Drinking on an empty stomach amplifies nearly every mechanism that makes alcohol hard on your kidneys.11
