Alcohol tremors are a common part of alcohol withdrawal syndrome (AWS). These tremors are an involuntary response that happen as the body adjusts to the absence of alcohol and are also called shakes.
This withdrawal side effect is the reason many people have shaky hands after drinking stops. The tremors are an indication that your body has become physiologically dependent on alcohol, and the withdrawal process has started.

Why Do Alcohol Shakes Happen?
Chronic alcohol use causes the body to become dependent on alcohol. Here’s what happens.
- The frequent presence of alcohol alters the brain’s release of GABA (gamma aminobutyric acid), a neurotransmitter that slows the neurological system.
- Alcohol also suppresses the production of glutamate, an excitatory neurotransmitter.
- Over time, the brain adapts to the presence of alcohol, so it no longer produces the normal amounts of these neurotransmitters.
- When alcohol suddenly is not present, the neurotransmitters “rebound” to try to stabilize the body.
- The rebound creates an overexcited central nervous system, which results in anxiety, high heart rate and tremors.

You are more likely to experience alcohol shakes if you have one or more of these risk factors:

How Long Do Alcohol Tremors Last?
While alcohol tremors are a common side effect of alcohol withdrawal, it’s important to know how long they should last and what to expect.
Alcohol Tremors Timeline
- Onset: 6-24 hours after last drink
- Peak: 24-72 hours
- Recovery: 3-5 days
During this time, it is common to experience sleep disturbances and anxiety. The combination of these symptoms can linger more than a week in some people.
This timeline can vary based on concurrent sedative use, any comorbidities present and your prior drinking patterns.
When to Seek Medical Support
It is possible for withdrawal symptoms to require medical attention. During the first six to 48 hours, the risk of seizures is highest.
Delirium tremens are also a risk, typically during days two through eight after your last drink. These conditions indicate a life threatening level of neurological hyperactivity and require immediate medical support.
When to Call 911
If any of the following red flags appear, or if the person cannot be safely monitored, call 911:
- Repeated vomiting
- High fever
- Seizure
- Confusion/hallucinations
- Fainting
- Chest pain
- Very high blood pressure or heart rate
When to Get Same Day Medical Care
Safe alcohol withdrawal often requires medical care. To ensure symptoms don’t progress to lethal levels, seek same day medical care for the following symptoms:
- Moderate tremor
- Anxiety
- Inability to sleep
- Heart rate above 100
Never “treat” alcohol withdrawal symptoms by drinking. This can worsen symptoms and increase the risk of seizure. Instead, prior to receiving medical care, take the following steps to stay safe:
- Avoid driving
- Take small sips of fluid to remain hydrated
- Arrange for constant supervision to monitor progress
Clinical Assessment & Treatment of Alcohol Withdrawal Shakes
Clinicians can assess the severity of withdrawal to determine appropriate treatment for alcohol shakes.
They use the Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar) to measure severity. This helps determine what, if any, medications should be prescribed.
CIWA-Ar
The assessment includes a rating scale for 10 withdrawal symptoms:
- Nausea
- Tremor
- Anxiety
- Agitation
- Sweats
- Orientation
- Headache
- Visual disturbance
- Tactile disturbance
- Auditory disturbance
The total score guides clinicians in treatment options.
- 0-9: Mild
- 10-18: Moderate
- 19+: Severe
Scores in the moderate range may indicate a need for medications to assist with withdrawal. Severe withdrawal is usually treated with medications.
Clinicians repeat the assessment to determine the trend of symptoms. They combine these scores with current vital signs and your medical history to guide treatment.
Withdrawal Medications
Several medication and supplement options are used for effective treatment of alcohol withdrawal and alcohol tremors.
Benzodiazepines: (diazepam/lorazepam) These are considered the first line treatment for alcohol withdrawal. They are prescribed using fixed dosing or symptom triggered dosing, to stimulate GABA receptors.
Adjunct medications may be used based on symptoms and medical history. These include:
- Carbamazepine/gabapentin to calm the central nervous system
- Phenobarbital to target GABA and glutamate receptors
- Beta-blockers/clonidine to treat autonomic symptoms
- Thiamine to restore thiamine deficiency
Additional support may be provided through antiemetics (nausea medication), hydration and electrolytes.
Choosing the Right Detox Setting
Detox programs are available in inpatient and outpatient settings. It’s important to undergo detox at the appropriate level of care. This chart indicates which setting may be best for your situation.

Elevate helped save my son’s life! One year ago, I was on a desperate search for a place that would help my son quit drinking. I went online and was so overwhelmed with all the choices! I called a couple places and was not impressed with what they offered.
I prayed to God He would lead me to the right place, and He did! The 3rd place I called, this nice guy named Josh answered, and right away, we connected. I asked questions, he asked questions. What I loved about what he said, was that they did not rely on prescribing a bunch of meds to “fix” my son, but offered an in depth program that included exercise as part of the journey to recovery, and to ultimately learn to not rely on anything to self medicate ever again.
Ding! I knew he would love something like this, and the more Josh talked, the more I wanted to get my son on a plane! Of course, the final decision was up to him, and at that point, he was ready for a major change, and trusted my judgement. Thank God!
Within days, my son was up north and that was the beginning of his new life because today marks his ONE YEAR from not having a drop of alcohol! In fact, he liked Elevate so much, he stayed up there, trained heavily for 3 months, and is now an employee who helps counsel new clients. He earned Employee of the Month last month and I couldn’t be more proud of the hard work he did and does for himself.

If I could give one suggestion, it would be to review the leadership on second shift — there are other staff members who show real diligence and could make a strong impact in that role.

This place quite literally saved my life. I struggled for many years with substance abuse. I tried for several years to get clean on my own but was never successful. I have nothing but great things to say about the Plymouth House if you are serious about wanting to heal yourself. Detox and Rehab are never an easy decision. You know that you want to better yourself but the process is never an easy one. The staff and clinicians at PH are very understanding non judgemental. I want to share my experience at Rise Malibu. I started 2 months ago for a 30 day inpatient treatment. In one word I would describe my experience as AMAZING!!
I have been to other facilities before but this experience is life changing. Its still early in my recovery, (60 days so far) so I can’t speak long term yet. My main therapist Dr Mark was incredible, and had approaches to sobriety that I had never heard of before. Terry, Michael, Cat, Gina, Tim and were all awesome therapists with differing methods of treating addiction. Not just AA which was enlightening to me. As a holistic sceptic, I was very surprised at how it helped me, especially the breathing techniques. I will utilize the breathing and yoga techniques the rest of my life. The support people at the facility John and Nam were great. Offering walks and hikes all the time as well as the Sunday outing. Especially enjoyed just talking about life with John who is a wonderful person and family man. The nurses Paige, Sherri and Zach were awesome to talk to and took very good care of me. The Chef Bryce is a 5 star chef who could work at the best restaurants. The food was great. The facility is beautiful with views of the ocean from every room and a pool on top floor where I stayed.
Ron, the owner is a great guy and he really does care. That’s why he does this. I talked to him for an hour on my first call to Rise and he actually answers the phone himself every time I called the number. Again, it’s his passion to help people. The staff started talking about an aftercare plan 10 days after I joined the facility and I had a fully set up aftercare plan complete before I even came home.
I very much look forward to writing back again in 6 months as I continue my recovery.
In summary I don’t think you can find a better place. I felt my like I was in a protective bubble the entire time with people who really cared about me.

This was my second tine in DFC outpatient treatment, the reason I chose DFC was their teaching, there, client, and the friendly environment. I have been to many treatment centers in my past when I knew in my heart I was serious about recovering from drugs and alcohol. There was no other place that I wanted to go other than Design For Change. They give group and individual counseling tailored for the individual which helped me tremendously. Today 8/1/2025 marks one year of sobriety. One year ago today I entered Elevate in Santa Cruz to start my detox from alcohol. A week later I relocated to the South Lake Tahoe facility. I had been drinking alcohol 45 plus years. I spent 30 days in Tahoe learning why and why I shouldn’t stay on my destructive path. Thank you to all that helped me. “OPP”

Calling treatment centers to explore options for a loved one was really overwhelming for me, but my family knew a must-have of any treatment program our loved one admitted into was a strong family education and support component. The Ridge made the information-gathering process so smooth and so reassuring. The staff answered every question we had, every question our loved one had, and they set us all up with support all the way around, right from the start. From drop-off and intake all the way through to discharge, The Ridge’s team was incredible. Their patience and guidance to / for my family throughout the entire process was truly invaluable. We feel a new level of confidence in our communication with and support of our loved one, as well as an entirely new understanding of the disease of addiction, that we know will set us up for success moving forward.
I don’t have words for the amount of gratitude my family and I have for The Ridge. Our loved one’s treatment process and newfound sobriety – along with their commitment and dedication to staying sober – would be enough in and of itself, but the rest of us had a life-changing experience as well. We learned so much, have knowledge and education around addiction that we didn’t have before, and have found an amazing community of people we’re so lucky to have found. I cannot recommend The Ridge more highly or more strongly for family members seeking treatment for their loved one’s addiction.
