Alcohol tremors are a common part of alcohol withdrawal syndrome (AWS). These tremors are an involuntary response that happen as the body adjusts to the absence of alcohol and are also called shakes.
This withdrawal side effect is the reason many people have shaky hands after drinking stops. The tremors are an indication that your body has become physiologically dependent on alcohol, and the withdrawal process has started.

- A hangover is the most intense when it begins and then improves throughout the day.

Why Do Alcohol Shakes Happen?
Chronic alcohol use causes the body to become dependent on alcohol. Here’s what happens.
- The frequent presence of alcohol alters the brain’s release of GABA (gamma aminobutyric acid), a neurotransmitter that slows the neurological system.
- Alcohol also suppresses the production of glutamate, an excitatory neurotransmitter.
- Over time, the brain adapts to the presence of alcohol, so it no longer produces the normal amounts of these neurotransmitters.
- When alcohol suddenly is not present, the neurotransmitters “rebound” to try to stabilize the body.
- The rebound creates an overexcited central nervous system, which results in anxiety, high heart rate and tremors.

Anyone who undergoes alcohol withdrawal may experience alcohol tremors.
You are more likely to experience alcohol shakes if you have one or more of these risk factors:
- If you are a senior
- A medical illness
- Experienced prior severe withdrawal

How Long Do Alcohol Tremors Last?
While alcohol tremors are a common side effect of alcohol withdrawal, it’s important to know how long they should last and what to expect.
Alcohol Tremors Timeline
- Onset: 6-24 hours after last drink
- Peak: 24-72 hours
- Recovery: 3-5 days
During this time, it is common to experience sleep disturbances and anxiety. The combination of these symptoms can linger more than a week in some people.
This timeline can vary based on concurrent sedative use, any comorbidities present and your prior drinking patterns.
When to Seek Medical Support
It is possible for withdrawal symptoms to require medical attention. During the first six to 48 hours, the risk of seizures is highest.
Delirium tremens are also a risk, typically during days two through eight after your last drink. These conditions indicate a life threatening level of neurological hyperactivity and require immediate medical support.
When to Call 911
If any of the following red flags appear, or if the person cannot be safely monitored, call 911:
- Repeated vomiting
- High fever
- Seizure
- Confusion/hallucinations
- Fainting
- Chest pain
- Very high blood pressure or heart rate
When to Get Same Day Medical Care
Safe alcohol withdrawal often requires medical care. To ensure symptoms don’t progress to lethal levels, seek same day medical care for the following symptoms:
- Moderate tremor
- Anxiety
- Inability to sleep
- Heart rate above 100
Never “treat” alcohol withdrawal symptoms by drinking. This can worsen symptoms and increase the risk of seizure. Instead, prior to receiving medical care, take the following steps to stay safe:
- Avoid driving
- Drink water and nonalcoholic fluid to remain hydrated
- Arrange for constant supervision to monitor progress
Clinical Assessment & Treatment of Alcohol Withdrawal Shakes
Clinicians can assess the severity of withdrawal to determine appropriate treatment for alcohol shakes.
They use the Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar) to measure severity. This helps determine what, if any, medications should be prescribed.
CIWA-Ar
The assessment includes a rating scale for 10 withdrawal symptoms:
- Nausea
- Tremor
- Anxiety
- Agitation
- Sweats
- Orientation
- Headache
- Visual disturbance
- Tactile disturbance
- Auditory disturbance
The total score guides clinicians in treatment options.
- 0-9: Mild
- 10-18: Moderate
- 19+: Severe
Scores in the moderate range may indicate a need for medications to assist with withdrawal. Severe withdrawal is usually treated with medications.
Clinicians repeat the assessment to determine the trend of symptoms. They combine these scores with current vital signs and your medical history to guide treatment.
Withdrawal Medications
Several medication and supplement options are used for effective treatment of alcohol withdrawal and alcohol tremors.
Benzodiazepines: (diazepam/lorazepam) These are considered the first line treatment for alcohol withdrawal. They are prescribed using fixed dosing or symptom triggered dosing, to stimulate GABA receptors.
Adjunct medications may be used based on symptoms and medical history. These include:
- Carbamazepine/gabapentin to calm the central nervous system
- Phenobarbital to target GABA and glutamate receptors
- Beta-blockers/clonidine to treat autonomic symptoms
- Thiamine to address thiamine deficiency
Additional support may be provided through antiemetics (nausea medication), hydration and electrolytes.
Choosing the Right Detox Setting
Detox programs are available in inpatient and outpatient settings. It’s important to undergo detox at the appropriate level of care. This chart indicates which setting may be best for your situation.

I checked myself in for alcoholism and opiate abuse. I was drinking so much I was falling and hurting myself and ruining my marriage and on a path to possibly loosing job and even land myself in jail. I was miserable and at rock bottom. Once there after the detox period i started attending group therapy sessions, individual therapy session’s sessions, and working with the techs to become involved in my recovery process. First 5 days i was detoxing and once that was over i got super involved in the program and day by day got better each day. I received excellent nursing and medical provider support. I had amazing group therapy sessions. I was fed delicious meals every day. I had a great case manager and had a primary therapist who was absolutely perfect. The staff was absolutely first class and my treatment started me on a successful path to recovery. This place literally changed my life. They took care of my every need physically, mentally, and emotionally. I highly recommend if you need help for addiction this is where you need to be.

This place has changed my life. I have been to rehabs before but I felt like this place is where I would get clean for the last time since i walked in the doors. The music assisted therapy was perfect for me and the staff is all amazing. The groups are really beneficial and really got down to the core of why i turned to drinking every time something upset me. My experience with All In Solutions has been a transforming experience from the moment they picked me up from detox. I walked in the door of their facility, broken, physically, mentally and spiritually. I was met at the front door by the intake coordinator. This lady completed my intake in a very welcoming, calming relatable and professional way.

The housing director explained to me what my housing was going to look like. The groups were facilitated by very professional, innovated, creative individuals that were not only relatable. Every employee at All In Solutions have been very kind, professional caring, giving people. Most of the employees are in recovery themselves, and even if they were not in recovery, they were still relatable, caring, giving people. I was only gonna stay for two weeks. Then extended to four weeks.Then I extended for six weeks at All In Solutions. I went from no Hope to seeing my life in a whole new perspective. All In Solutions gave me purpose hope perspective! I love what this place has offered to me…I highly recommend this facility to the sick and suffering addicts, I promise your life will never be the same like they say “it work if you work it”…. Very clean, staff is nice and kind….what ever you need to succeed they got you. It’s like living at home….they need to have these facilities all over the world. I completed 86-days at Hawaiian Island Recovery, and it was genuinely one of the most effective and supportive treatment experiences I’ve had. The house managers are compassionate people who truly understand what residents are going through. Most notably, my case manager Chevone was outstanding. She is highly competent, deeply understanding, and acted as a strong advocate for my needs throughout the program. She was a keystone figure in my time at HIR, coordinating care, pushing for what was truly helpful, and keeping everything aligned with my recovery goals.

HIR was exactly what I needed! I was at an absolute low point in my life and this place completely turned things around. The staff was extremely friendly and caring, some have years of recovery themselves so they’re also very easy to relate to. The meetings they took us to were extremely helpful and even fun! Delicious home cooked meals are prepared everyday by a wonderful Chef, yum. I stayed 30 days but wish I could have stayed the full 90. I still go back on alumni days and Big Book Studies to stay in touch with everyone there, I love HIR!

I did 60 days at the residential program and it genuinely changed my life. This wasn’t my first time in rehab I’d been to two other facilities on the mainland that didn’t stick. What was different here is that they built my treatment plan around me specifically, not a one-size-fits-all schedule. The therapists actually listened. The staff-to-client ratio is small so you’re not just a number. Frank and Fernando were incredible to work with during individual and group sessions. Being on the Big Island away from my usual environment and triggers gave me space to actually focus. I’ve been sober since completing the program and I credit this place for that.

The staff truly cares about their patients and provided care that met my needs. The whole program exceeded my expectations and I can’t thank everyone enough for the impact they made on my life and mental health. Deciding to seek treatment was not an easy thing for me but I am so glad I chose Footprints to Recovery. It wasn’t all just sitting around doing therapy all day and we got the chance to do yoga, art and music therapy, and also go outdoors with Adventure Recovery. Footprints was an absolutely stellar facility to go to after completing inpatient addiction treatment. The staff were all very patient and understanding during my biggest ups and downs during treatment, and I never felt any judgement even at my lowest points.
Recovery is such a serious rock to carry but the groups and providers helped me collect the tools necessary to be able to tackle this journey on my own.
The nursing team was incredibly helpful in finding me medications that are actually helping my mental health and addiction cravings. I’ve had more success in the three months I got to share with Footprints than the years I’ve worked with my primary care and therapist.
Andrew with case management is hilarious and hugely helpful with resources and managing work and life alongside treatment.
Anika is an incredible therapist and I am sad to leave her care, but know she will continue to do amazing things for other clients.
If you are struggling with any substance abuse, I highly recommend seeking help with Footprints. They will absolutely get you back on your feet.
