Transformation House - North Ferry Street is a rehab facility in Anoka, Minnesota. This residential center provides care for adults who have substance abuse and alcohol issues. Traditional therapy and education classes are provided to help improve your health. They also provide a descaling program so you can transfer to outpatient, recovery housing
I can't say enough good things about Transformation House. My experience here was a night and day difference from my previous treatment center. My counselor was incredible – so empathetic and understanding, which made it easier to be honest and work through things. The entire staff genuinely cares about your well-being. It's clear they're not just going through the motions. What I appreciated most was that they focused on helping me find the right path forward, even after I left. The outpatient program they connected me with was fantastic. Today, I'm proud to say I've graduated, I'm in college, I have a job, and most importantly, I have my kids back in my life. My advice to anyone going to Transformation House is to be honest with yourself and your counselor, and truly commit to the process. Stop making excuses and put in the work – they will meet you where you are and provide the support you need. I also built some strong connections with the other women there. The staff creates a supportive environment, but ultimately, your willingness to change is key. Transformation House truly changed my life, and I'm incredibly grateful for how far I've come.

I went here when I was an adolescent I think when I was around 16, I’m 21 now. Growing up I didn’t go to a lot of humane treatment centers when I think back on my time here though it was awesome. The food was great I was never hungry we went on field trips and got outside often. It was so fun here it felt like summer camp but in a therapeutic way. I hope it’s the same since I went there. Thank you guys.

I’ve been receiving services from Nystrom and Associates for many years. Their attention to your needs is always focused on, and I feel like their staff is well trained to handle any situation one might struggle with. They are very open to the LGBTQ+ lifestyle, and I didn’t feel ostracized in any way. Each therapist has always been kind, considerate, respectful, and knowledgeable. ⭐⭐⭐⭐⭐ I can't say enough good things about Nystrom and Associates in Eden Prairie! I've only had two therapy appointments so far, but I'm already looking forward to scheduling my next ones. Heidi Bishop has been absolutely wonderful! She creates a safe and supportive environment that makes it easy to open up about my feelings and experiences. I'm hopeful that our work together will help me navigate the challenges I'm facing. I also had a Rule 25 assessment with Julien Monroe, and she was fantastic! She was not only sweet but also incredibly efficient with her time. It was clear that she genuinely understood my situation and communicated everything in a way that made me feel comfortable and heard. I appreciate the variety of services they offer, including my upcoming telehealth appointment with an ARHMS worker. It's great to have multiple ways to connect, making it convenient to fit therapy into my busy schedule. Plus, having so many resources available in one place makes it easy to manage all my needs and appointments. Overall, I highly recommend Nystrom and Associates. The staff is caring, professional, and truly invested in helping their clients. I’m excited to continue my journey with them! Thanks! —Jerzee

Gwen and Scott are the best. I tried other treatments, but this one is definitely the best. All the other ones I tried, were for profit, and it truly showed, they were more interested in moving you to a higher level of care than actually helping you. Would totally recommend this place to anyone that is willing to take the next step forward. Being obedient to Christ and last but not least listen to staff. God created authority. All authority comes from God anyone that rebels/disobeys authority is disobeying God. It's not the program that will change you it's you allowing God to transform you while you're in the program. If you're not ready to stop doing what isn't working you won't be ready to start doing what is working. Be blessed

I made it to stage 4, but I was just as miserable and just as abused as everyone else. I believe the only reason I got stage 4 is because I started to isolate myself which meant I stopped advocating for other people and myself. I came into Avanti outspoken and morally strong but I left broken and quiet. They don’t want you to have a voice and make you think you are nothing more than your struggles. They lie on treatment plans so “insurance doesn’t stop paying” but really it’s because they make money off of children’s pain. A few staff quit because of the way we were treated but they didn’t report the abuse to anyone. They failed us just as much as the owner of avanti. I left twice as traumatized then when I went it. I’m still trying to find my voice again 4 years later. I could write a 500 page paper on everything I went through at 6 months in avanti. Please do not send your children here, and I’m so sorry to anyone who is reading this after staying at avanti. I talk to 3 people I met there 4 years ago and we all still regularly recall the things that happened to us at avanti. What avanti does to children will never be legal or morally okay. Every single mental health professional I’ve told this year that I went to Avanti gets a look on their face or says something negative about it. This place is so scary, I don’t understand why no one has done anything.

I’ve been waiting to make this review… THIS PLACE IS HORRIBLE! & that is a understatement.. If you have any love for your child DO NOT send her here. Please. This place is disgusting, traumatizing, and belittling. I was at avanti for a little over 5 months, somewhat close to discharge because the programs stay was an estimated 6-8 months I believe. They kicked me out after residents threw a huge crisis… every crisis was blamed on me. EVERY CRISIS!!!!!! I swear they were racist. They favorited certain residents and didn’t give a single crap about the rest. Therapists were absolutely terrible and so were the staff. All the people you were supposed to confide in with your struggles there were unreliable, rude, and negative. Residents got one 10 minute phone call a day which was never guaranteed because of crisis and staff being lazy. Which made things lot harder for me. It was hard enough being away from home but barely getting the opportunity to speak to my support system made things a lot more difficult. The handbook was NEVER followed ,rights were never respected. I spent most of my stay in a place we call fireside (which was a room we got sent to when there was a crisis for safety) then I did learning skills to get myself better to have the ability to discharge home. I could go on for DAYS about this place and how messed up it was. I came out with more trauma then I went in with. If I could give this place a negative star review I would. x1000. SHUT THIS PLACE DOWN!!!!!! I’m now an adult. & have learned to live with what this place did to me mentally. I graduated high school and plan on going to college to major in criminal justice & minor in political science eventually. I WANT TO ADVOCATE! I am living in my own apartment, working 16 hour shifts, driving, and I’m expecting a baby boy in March. I hope every Avanti staff sees this review .. & remembers the vulnerable 16 year old girl that they traumatized and belittled. I hope the girls who were also sent here have found peace and got the help they were looking for. SHUT THIS PLACE DOWN!!

If I could remember everyone’s names from start to finish of our 24 hour stay I would certainly give thanks to them all. Every single staff member we came across was beyond kind and helpful. My daughter and son had amazing experiences and they actually have BRAGGED about surgery to their little 4 and 2 year old friends. You guys made everything so easy. I was beyond impressed with everyone however I’d like to give one specific shout out to Megan a PICU RN that was spending her time in the Rehab unit. She was our first nurse to attend to our recovery room with my son for his 24 hour stay. My son loved her and she was GREAT with him. Made sure she got to know him, showered him with love, and constantly was his cheerleader. Hoping she receives some sort of recognition or more for her excellent patient care.

When one pays premium price for health care (out of pocket) ito examine and correctly diagnose a medical condition (possible fracture or broken bone in this instance), if the clinician is not a medical doctor, this should be clearly conveyed to the client so they decline service and go elsewhere where medical doctors are employed. I was not seen by an MD, rather a DO, who did not know anything about my injury and merely referred me to orthopedic Drs. at clinics I told him I would NOT go to since I already have an orthopedic Dr. with whom I am working. He did not even manipulate the joint or try to better determine the nature of the injury. He could not even enlarge the digital Xray that he ordered, in order to determine if there was a dislocation or fracture. I also asked for a prescription for a persistent cough, which was supposedly verified by the DO listening to my lungs through a stethoscope through two layers of thick cotton shirts, not by direct contact with my skin. Credible, scholarly medical articles found on line indicate the unreliability and therefore the efficacy of such practice. I did NOT get any measurable amount of medical care or attention that is worth the price that FAIRVIEW Clinics will attempt to bill. I'm fighting this one. In any other sphere of commerce, if a business does not deliver, there is little or no charge. The health care industry should be no different.

Willow Haven didn’t just give me a place to stay, it gave me space to heal. From the moment I walked through the doors, I felt like more than just another name on a check-in sheet. The atmosphere is peaceful and comfortable, a true home setting that immediately puts your heart at ease. The meals? Five-star dinners every night. You can taste the love in every bite. What makes Willow Haven stand apart, though, is the balance. The schedule is the perfect blend of structure and free time, just enough programming to challenge and grow, and just enough space to breathe, reflect, and rediscover who you are. The staff? They are the heartbeat of this place. A rare blend of inspiration, guidance, and love — the kind that speaks into your life without judgment and walks beside you until you can walk on your own. They don’t just teach recovery. They live it. And they pour themselves into every person who walks through the doors. You just have to be willing to receive it. Willow Haven is more than a facility. It’s a turning point. A sacred pause. A reset you didn’t know your soul needed. Forever a blessing in my life. I walked in broken, I’m leaving restored.

Excellent service, excellent attention. I think the wait is normal because I've been to others and the wait is the same or longer, but as of today, November 2025, the service is 10/10.

Tanya is the worst employee / counselor ever. Sends my girlfriend to a halfway house where with out me she will have zero chance of staying sober. Way to go NorthStar! Nice lies you tell also just to not cause concern. Lies lies lies! NorthStar Regionals ONLY area of expertise is in lying to their patients and patients loved ones. They are really good at ripping up good healthy relationships. Tanya is very rude and unprofessional. Tanya will judge you immediately and base clients recovery on her horrible bias misplaced judgements. STAY AWAY FROM TANYA AND THIS PLACE ALL TOGETHER. ANY OTHER RECOVERY CENTER OTHER THAN NORTHSTAR AND YOU SHOULD BE GOOD. NOT HERE. MAYBE IF TANYA IS FIRED THE PLACE HAS A CHANCE
